We are looking for an enthusiastic, results focused individual who is passionate about making a difference to people's lives to join ourdynamic, supportive and busy team within the Public Health Directorate at Hywel Dda.
As Service Development Manager for Complex Needs you will establish and lead a virtual, multi-disciplinary complex needs team across organisations delivering measurable patient and population outcomes. You will establish and lead, with the Head of Population Health, the complex needs project board, in order to improve outcomes for individuals with mental health, substance misuse and housing / homelessness issues.
Main duties of the job
You will play a pivotal role in forging partnerships, leading strategic direction and developing and modernising service delivery. Including leading interventions across prevention, early intervention, treatment and recovery for individuals with complex needs. This will involve development of project implementation documentation, service pathways and the ability to define and measure clear output and outcome measurements in collaboration with partner organisations.
You will work with internal Health Board and external partners to ensure the programmes are aligned with best practice and are visible as well as tailored to reduce health inequalities and improve health and wellbeing.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 12,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:
Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest

- Educated to Masters level or equivalent level of experience within a relevant organization
- Evidence of in depth knowledge of the Complex Needs agenda (drug and alcohol , mental health and housing / homlessness) agenda and interfaces
- Knowledge of public sector partnerships and public sector working
- Considerable knowledge of patient facing service delivery across NHS and social care, housing / homelessness environments

- Public Health qualification

Experience

- Substantial experience within the drug and / or alcohol field, housing/, mental health or associated field
- Substantial experience of successfully leading and developing operational service delivery within a complex, multidisciplinary environment for patients with complex needs

- Signifcant experience of developing patient and service pathways, risk management and service evaluation
- Exprience of developing needs assessments and evaluating a range of data / service information to inform plans/ produce lessons learnt reports, set priorities, establish outcomes and make recommendations outcomes
- Change management experience (commissioning, , service development, people management etc) within the NHS, local government or comparable organisations
- Experience of working in partnership at a senior level which has resulted in service improvement
- Experience of writing reports with clear recommendations, programme planning, business case processes and communication on muti agency workstreams.
- Experience of negotiating and influencing priorities with partners as part of the development and review of policies, services, plans and strategies
- Experience of successfully managing change and innovation with demonstrable results
- Experience of managing a diverse workload in a high pressured environment and ensuring that deadlines are met
