An exciting opportunity has arisen for an advanced clinical practitioner to support the expansion of the Perioperative service at the RDUH. Perioperative medicine for older people undergoing surgery has rapidly become an essential surgical support service and is recognised locally and nationally as being of high quality, innovative and clinically effective.

We are looking for a compassionate and experienced registered health care professional with experience of Healthcare for Older People. The individual will work autonomously as an Advanced Practitioner within the Perioperative team and provide a high level of expertise to ensure patient-centred clinical care for the complex and high risk Surgical patients. They will exercise advanced clinical expertise, levels of judgement, discretion and decision making whilst undertaking the skills of assessment, examination to provide a diagnosis and formulate timely treatment plans within an agreed scope of practice for patients on the Surgical pathway. Evaluate investigations and revise treatment plans according to patient need and assess impact and outcome. Provide expert professional advice to patients, carers and colleagues. Monitor and lead improvements to standards of care through supervision of practice, clinical audit, implementation of evidence-based practice, teaching and support of colleagues and the provision of professional leadership. The Perioperative team including the Advanced Practitioner will support a holistic care approach to patient care including the medical, surgical and social aspects. Medical diseases and complications have been identified as major factors resulting in poor surgical outcomes and prolonged or complex perioperative pathways. Identifying and optimising these acute or chronic medical conditions early, are shown in multiple national and international studies to reduce morbidity, mortality and length of stay.

Working Pattern: 37.5 hours per week.
