To devise, implement and follow up specialist treatments across the disease spectrum for patients within a specific group.

To be a skilled practitioner providing clinical care using an extensive theoretical knowledge base and practical experience to underpin the role, while acting as a role model for the delivery of care.

To deliver specialist educational programmes to a range of healthcare professionals in relation to the specific disease including medical, nursing and Allied Health Professional staff.

Main duties of the job

An exciting opportunity has become available within the Cardiothoracic directorate. We are looking for a motivated and enthusiastic individual to join our Cardiac Rehabilitation team as a Band 6 Specialist Nurse. The role is challenging, rewarding, diverse but also extremely rewarding. The service is Nurse led and provides high quality centered care for patients in Cardiff & Vale post MI, ACS and Cardiac surgery.

Successful candidates will be required to manage their own caseload, act as a keyworker and liaise across other specialities as part of the Cardiac Rehabilitation MDT.

The role of the Specialist Nurse provides opportunity for personal and professional development and we are seeking individuals who can work autonomously and as part of a small supportive and highly knowledgeable MDT.

Our services are delivered across UHW, UHL and the Community including Community visits so Candidates must have the ability to travel between sites.

Qualifications

Essential

  • First Level RGN
  • Degree in Nursing or equivalent
  • Evidence of current post registration development


Desirable

  • Health Promotion/councelling training


Experience

Essential

  • Sufficient post basic registraion with extensive knowledge in Cardiac Nursing
  • Experience managing own caseload
  • Experience in teaching and providing professional support


Desirable

  • Health Promotion education or rehab experience
