Band 6 Home First Physiotherapist, Newcastle, Staffordshire
A fantastic opportunity has arisen for full time Band 6 Home First Physiotherapist to be based at Bradwell Hospital.
We are looking for an individual who is passionate about achieving the best patient outcome to meet the patient’s plan of care in their own homes.
The Home First Team is an integrated multi-disciplinary team that provides short term, rehabilitation and health interventions to enable people to maximise their independence and meet their health and social care needs within North Staffordshire and Stoke on Trent.
The service is responsive to individuals requiring health and social care assessments in the patient’s own home following the national discharge to assess model of care. The team implements plans of care and interventions to maximise an individual’s health and wellbeing.
The Home First Team delivers a high quality service where all staff members promote the philosophy of independence, to support timely hospital discharge and to prevent inappropriate hospital admissions.
The service operates from 09:00 to 20:00, 7 days per week, with early and late shifts on a rota basis.
Flexibility is required to work across other localities as required to meet the capacity and demand of this urgent care service.
Due to the nature of the Home First team it is essential that applicants have the ability to travel independently in order to cover all bases in Newcastle, Moorlands and Longton as and when required.
