We are looking to recruit an experienced, motivated individual to join our complex discharge team. You will be predominantly based at Charing Cross Hospital site but will be expected to work across site as required.

We are looking for individuals with strong communication and interpersonal skills - highly motivated, with initiative and receptive to feedback. You will have critical thinking skills and be able to work well as part of a team.

The post holder will:

Promote and work within the Discharge to Assess Pathway working collaboratively with patients and their families

Work collaboratively with the ward teams to identify and assess patients care needs and co-ordinate services to support the safe and timely discharge of patients with complex needs, liaising with the multi-disciplinary team and partner agencies across acute and community settings

Hold a case load of complex patients

Participate in multidisciplinary meetings, ward based long stay meetings and other meetings as required representing the complex discharge team and acting as a role model

Be expected to actively participate in the education of your peers and other professional groups within the Trust

Main duties of the job

The aim of the Specialist Discharge Nurse role is to deliver high standards of patient care and pathway management in order to ensure a timely, safe and patient centred discharge.

Education/ qualifications

Essential

  • Registered Nurse
  • Teaching qualification or equivalent experience in hospital / community team setting
  • Knowledge and able to demonstrate current National Policies relating to the field


Desirable

  • Relevant specialist course


Experience

Essential

  • Demonstrable experience in acute setting
  • Multidisciplinary working
  • Experience with supporting Homeless and Overseas clients


Desirable

  • Experience / understanding of complex discharge systems in an acute setting
  • Experience of interagency and joint working


Skills/knowledge/ abilities

Essential

  • Good communication skills
  • IT skills including Excel, Word and database systems
  • Demonstrates the ability to adapt to NHS service developments and new theories for use in the clinical area
  • Self-confidence to make informed decisions
  • Has basic literacy and mathematics skills


Desirable

  • Demonstrate commitment through involvement in Trust wide initiatives
