We're an integrated community therapy team whose aim is to help the local population maximise their abilities to improve their quality of life. We have an emphasis on prevention, early intervention and long term self-management to enable patients to live better lives at home. You will play an integral part of the Community Rehabilitation Team, managing a diverse community caseload including orthopaedics, frail elderly, post-surgery and neurological long-term conditions. You will have opportunities to support students and be involved in regular training and peer support. We are part of an innovative neighbourhood model with close links with district nurses, mental health teams, community agents and adult social care staff as well as the rapid response and hospital at home teams.
Main duties of the job
Assessment and treatment of adults with a rehabilitation need that can be met in the community. Maintaining peoples independence and reducing hospital admissions.

We have a lovely team at Burnham that I feel has great teamwork at its core. All members of the team are very supportive and flexible and are able to adapt to the needs of individuals and the wider team where needed. We communicate well as a team and I feel I am freely able to communicate any issues or concerns within my immediate team and to my manager. I like to think we are all happy to share our skills and knowledge with each other and have regular clinical discussions on a regular basis to help problem solve any issues/questions we have regarding patients on our caseload (Current Therapist in the Community Rehabilitation Service , Burnham on Sea)
