Band 6 Practitioner-Led Clinic (PLC) Mental Health Practitioner

Are you an experienced community mental health practitioner wishing to develop your career in the community but looking for a different role to care coordination and caseload management? Are you able to practice autonomously and have the courage to seek ambitious recovery goals for our service users?

Southern Health NHS Foundation Trust are looking for a full time Band 6 Mental Health Practitioner to work with clients in our new Practitioner-Led Clinic (PLC) in the East Community Mental Health Team, Southampton.

Many users of our service require evidence-based and collaboratively planned secondary mental health care but do not need the level of intervention and frequency of contact provided by care co-ordination (CCO).

Practitioner-Led Clinics are an effective outpatient appointment clinic alternative to CCO allocation with the emphasis on recovery, hope and engagement with the wider community, often with transfer of care back to primary care. They require highly competent senior clinicians capable of establishing therapeutic alliances with patients, co-producing comprehensive care and support plans with clear objectives and the ability to drive balanced risk decisions that promote independence and recovery.

Our PLC practitioners will be working with specific client groups, who are recovering through different pathways. Some service users will require time-limited brief psychologically informed interventions leading to discharge. Other patients receiving long term input such as clozapine treatment or depot therapy may require much longer term review and management within the clinic. This will make the work varied, exciting and satisfying to deliver.

PLCs are becoming a mainstream model of care delivery in many branches of physical healthcare and are increasingly prevalent in community mental health settings enabling the service to provide a graduated range of care intervention that promotes self-efficacy.

The posts are permanent representing the Trust’s commitment to exploring new ways of delivering community-based mental healthcare, and as such you will be working with a dedicated Band 7 Clinical Lead for the PLC ensuring the quality of care provided is excellent and patient flow in and out of our PLC remains fluid.

If you are a qualified RMN, Social Worker or Occupational Therapist with a strong clinical background and willing to develop an important new model of care in our CMHT we would love to hear from you.
