This is an exciting new role for an existing Band 6 nurse or physiotherapist or a Band 5 who feels thye would like a challenge within a new service.

The Respiratory Virtual ward within Worcestershire Acute Hospitals will aim to provide care to patients over a short period of time either on discharge from hopistal or as admission avoidnace. Initially this would be for COPD, but with the aim to expand to all respiratory conditions.

Main duties of the job

You would be working within a small virtual ward team which is part of the wider hospital and community COPD team, seeing patients in hospital, at home and reviewing them virtually.

Job description

Job responsibilities

Clinical Practice:

Will undertake specialist respiratory assessment and treatment of patients needs within hospital, virtually and in home environment using clinical knowledge and clinical skills.

Identify suitable patients for early supported discharge or admission avoidance, plan and arrange discharge and provide individualised, holistic care plans to suit the patients needs.

Make daily contact with patients on the virtual ward either via DOCOBO virtual platform, telephone or undertake home visits to patients who are on the virtual ward

Ensure all documentation and communication is maintained to a high standard

Manage a caseload of patients on the virtual ward.

Establish and maintain effective communication with patient and carers/relative and whole virtual ward MDT.

Recognised changes in patients condition that require the intervention of others and refer on as appropriate.

Deputise for band 7 when on leave.

Educational:

Deliver specialist training and educational programs for nurses, doctors, physiotherapists, and students, related to respiratory care, in order to develop the skills required to deliver safe respiratory care and promote a wider understanding of respiratory and the principles of its treatment

Act as an expert resource for health care professionals working with respiratory patients within the primary and secondary care settings, to advise about appropriate delivery of care to those patients

within the Trust for medical, nursing and other health care professionals

Identify and promote respiratory care related potential learning opportunities for other nursing staff

Takes responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with NMC/ HCPC requirements.

Participate in appraisals and setting objectives with junior staff in the department

Patient / carer Support / Involvement:

Provide education and advice for people with respiratory conditions helping them to understand the disease process, its management and potential coping strategies

Provide emotional support for patients who face a diagnosis of chronic or life limiting respiratory disease at the time of diagnosis, through the course of the disease process and during the final stages of their illness

Support patients, their families and other carers to influence and participate in decisions concerning their care by providing information on the services available for general and specialist information and care including specialist support groups

Having due regard for patient confidentiality, act as an expert resource for carers of respiratory patients, providing them with advice and support as required to improve their skills and confidence in the delivery of community respiratory care

Develop and update written information leaflets for patients, liaising with clinical governance and the patient information group

Leadership and Management:

To be deputise for Band 7 for the day-to-day operational line management of their direct reports

Be responsible for assessing and recognising emergencies in the specialty, acting as an advocate for the patient

Act as a role model and support for staff on respiratory and medical wards

Actively participate in the multi-disciplinary ward meetings

Act as an expert resource, providing clinical leadership and facilitating professional development amongst the multidisciplinary team

Promote best practice, using local and regional benchmarking groups for the generation and promotion of standards and clinical guidelines

Liaise with departments within the hospital to address specific respiratory condition related issues, e.g. oxygen management

Participate in relevant groups and forums representing the interests of the Respiratory department as appropriate

Personal / Professional Responsibilities:

Be proactive in and maintain their own professional development in accordance with NMC requirements with due regard to maintaining the level of skill, highly developed specialist knowledge and expertise required to fulfil the specialist nurse role, including maintaining membership of relevant professional bodies and interest groups

Maintain clear and concise records of the patient's management in accordance with NMC guidelines and Trust standards

Promote and maintain good working relationships within and outside of the Trust, utilizing opportunities to promote the specialist service, and the work of the Trust

Contribute to the creation of a care environment and culture that delivers a consistently high standard of patient-centred care

Research and audit:

Keep up to date with published research and audit findings, critically appraising outcomes and using them appropriately to develop best practice and improve patient outcomes

Take part in local and national respiratory related research, in accordance with the Good Practice Guidelines, NMC guidelines and local policy

Undertake nursing research and publish the outcome, updating own knowledge to promote excellence in clinical practice

Participate in and be aware of clinical trials for patients

Clinical Governance:

Act at all times in accordance with the NMC code of conduct and local policies and guidelines, to maintain high professional standards based upon the best available evidence and research

Participate in the development of associated guidelines within respiratory care and other policies where individual clinical experience can be of value

Disseminate national and local guidelines to staff to ensure evidence based care. Within the respiratory team develop clinical guidelines and protocols and patient pathways

Strategic / Service Development:

Contribute to the operational and strategic development of the Virtual ward service within the Trust, defining and maintaining evidence-based standards of care, the quality of which fulfil clinical governance requirements

Apply Quality Improvement techniques to service development

Support staff in promoting delivery of excellent care and provide a clear focus for supporting and developing clinical leadership.

To facilitate the development of mechanisms for monitoring through put and uptake of service

Person Specification

Experience

Essential

  • Demonstrable hospital experience successfully delivering clinical care to general and specialist areas, including respiratory medicine, as a band 5 or above
  • Experience of applying physical health assessment skills in a hospital or community setting to respiratory or other patient groups
  • Experience of conducting and presenting research or audit data at a local level
  • Experience of contributing to the clinical training and development of staff


Desirable

  • Demonstrable management experience
  • Community Experience within Respiratory

Qualifications

Essential

  • Registered Nurse/ AHP with the relevant UK professional body with a first degree
  • Post-basic specialist qualification relevant to specialty
