To support the Primary Care & Community Nurse education agenda- ensuring cognisant of both current and future nursing lead requirements

The post holder will provide clinical skills training and information sessions aimed at up skilling the registered and unregistered workforce across the Division and the independent sector, in order to reduce avoidable hospital admissions.

The post holder will be expected to independently deliver training sessions when required. The post holder will be expected to support the safeguarding agenda across the Primary Care and Community Division, working cohesively with the Safeguarding, Deprivation of Liberty and Complex Care teams.

The post holder will help to drive nurse education forward and will support the provision of clinical training across a wide range of health conditions and a wide range of services where nursing care is deliveredsuch as GP practices, prisons and nursing homes.

To support the identification and evaluation of innovative solutions to local and strategic issues as they relate to nurse education and professional development, taking account of the conflicting demands of the service and complex facts to analyse, interpret and evaluate a range of options.

The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

Teach on clinical foundation courses and other professional development courses

To work closely with NCN Boroughs, palliative care, OOH, WAST and other relevant care teams to support the development of Advance Care Planning and hospital avoidance in Care Homes and across the community to support care closer to home/Prudent Healthcare, end of life care strategies and the Primary Care Plan for Wales

To deliver training programmes for clinical skills in response to identified learning needs for the Primary Care & Community nursing workforce , practice nursing workforce and independent sector registered and unregistered workforce

To support the identification and facilitation of professional development needs of all staffing groups across the registered and unregistered workforce, including Practice Nursing in response to service developments.

To liaise with the Community Senior Nurses, Senior Nurse Prison healthcare, Practice Nurses, GPs, Advanced Nurse Practitioners, Independent Sector Matrons and non-clinical care homemanagers to ensure that practice development is targeted towards patient's needs.

To ensure that training delivered is aligned with patient and staff need, and that service recommendations are actioned through education and responded to in a timely way

Person Specification

Registered Nurse


  • Advanced use of Office IT Systems
  • Teaching qualification or equivalent demonstrable level of knowledge and experience.
  • Educated to Degree level plus Specialist clinical knowledge to postgraduate diploma level equivalent.


  • PGCE
  • Experience of change management.
  • excellent presentation skills



  • Experience of dealing with confidential and sensitive data and appropriate maintenance and storage of records


  • significant experience at band 5 and above



  • Knowledge and understanding of the quality and patient safety agenda
