An exciting opportunity has arisen for a dynamic, motivated, Nurse or Pharmacy Technician to join Neath Port Talbot Medicines Management Team for Domiciliary Care.

Thesuccessful applicantwill be part of the integrated, multidisciplinary Community Resource Team who provide support and education in medicines management, specifically in domiciliary and intermediate care settings. This role aims to improve patient outcomes and minimising harm, including avoiding unscheduled care, admissions, and delays to transfers of care. It is ideal for individuals interested in integrated working and are committed to developing and expanding their skills to achieve the highest quality care and outcomes for patients in their own homes.

Main duties of the job

The role includes:
  • providing medicines management interventions to patients within their own home, involving lone working without direct supervision.
  • liaising with a wide range of Health & Social Care professionals to identify solutions for patients in a multidisciplinary network with a focus on promoting independence, prevention and early intervention.
  • delivering medicines management training to relevant staff groups.
  • managing a defined caseload within the speciality, using evidence based/patient centred principles to assess, plan, implement and evaluate interventions, maintaining records.
  • providing a lead contact between community pharmacy and social services with respect to medicines management issues, including the provision of regular updates, training and visits to community pharmacies to support the Medicines Administration Record scheme.

The post holder must be able to travel around the locality in a timely manner.

Person Specification

Essential Qualifications & Knowledge


  • Registered with relevant professional body such as NMC or GPhC
  • Specialist knowledge acquired through health related qualification such as BTEC science (pharmaceutical) Registered Pharmacy Technician, equivalent level qualification plus specialist training and experience to degree or equivalent level
  • Knowledge and evidence of Professional requirement for ongoing Continuing Professional Development to maintain knowledge and expertise


  • Further post graduate management or clinical qualification

Essential Experience


  • Post qualification experience
  • Experience of facilitating change to improve clinical practice
  • Experience and good working knowledge of clinical audit

Essential Aptitude and Abilities

  • Ability to define, collate, analyse and interpret data in a timely manner to solve complex problems and plan effectively, and to motivate people and facilitate change
  • Ability to provide medicines related information to a range of health professionals, managers, social workers, professional carers, and other health care staff, overcoming potential barriers to understanding
  • Ability to provide medicines related information patients and relatives, requiring well-developed empathic and reassurance skills
  • Effective interpersonal, communication (both written and oral), presentation and influencing skills


  • Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading, and writing in Welsh



  • Ability to travel within geographical area
  • Able to work hours flexibly
  • Ability to deal with occasional emotionally distressing situations such as counselling patients regarding their medicines or complaints with difficult situations and potential conflicts
  • Ability to work independently and effectively with a high degree of motivation for long periods and manage own unpredictable workload
  • Ability to utilise information technology, including Word, spreadsheets and PowerPoint
