An exciting opportunity has arisen to join the Mid Specialist Dementia and Frailty pathway team within Mid Essex. We are looking to recruit a motivated, hardworking team leader who has excellent communication skills with a passion for driving service improvement and working collaboratively with other health care providers and third sector agencies.

The successful candidate will be responsible for leading the Chelmsford team, ensuring the delivery of a high level service providing a proactive assessment, diagnostic, monitoring and management of patients with dementia of any age and people over the age of 75 years with complex Mental Health needs who have an age related frailty.

We are looking for:
  • An experienced registered mental health nurse
  • A competent, confident and proficient practitioner
  • Who is organized and able to work creatively and effectively in a busy and demanding role

It is essential that you have a UK driving license and able to drive for this position.

Main duties of the job

Provide leadership of the Multi Disciplinary Team to deliver the service

Responsible for the overall delivery of a range of evidence based interventions to service users based on identified holistic needs, review the effectiveness of implemented care packages and discharge service users back to primary care / other providers in a timely manner.

Develop robust interagency relationships with key stakeholders supporting development of the integrated care pathways and locality working.

Responsible for the coordination and management of the team in providing a safe, effective and therapeutic clinical service, ensuring the delivery of high quality community mental health services.

Responsible for general service management including
  • Appraisals
  • Supervisions
  • Complaints/investigation management
  • Management of sickness absence/disciplinary/capability and other workforce procedures and policies.

Support others in their learning and development in relation to the management needs of patients with dementnia and serious mental illness with age related frailty

To ensure the service works in line with CPA/Social carer/risk management/ safeguarding and other relevant policies.

Person Specification



  • Ability to conduct complex assessments demonstrating knowledge of the complexities of dementia and complex mental health needs in older adults and frailty


  • Knowledge of working with external agencies to support the service user group

Skills/ Experience


  • Good oral communication skills based on fluency on the English language


  • Provide clinical expertise and support to the Commissioning Panels (Health & Social Care)



  • experience of working with patients with dementia or other mental health problems


  • experience of working in a community setting as a Band 6 practitioner or equivalent

skills and Experience


  • Working knowledge of CPA, MHA, MCA,
  • Good level of IT proficiency


  • Understanding of the principles of the Care Act
  • Non Medical Prescriber
