An exciting opportunity has arisen for a physiotherapist to be part of a project which aims to support improvements in health promotion and advice provision to adult mental health secondary care service users.

Working within two CMHTs, the OT Team, a Dietician and a Physiotherapist will work collaboratively, to provide one to one interventions and group work to help improved the physical health of people living in Denbighshire with a diagnosed serious mental illness, with the aim of having improved physical health outcomes for this population .This will include advice on healthy eating, physical health and exercise, good mental wellbeing and practical skills and activity groups such as hiking and cookery, encouraging patients to develop healthier lifestyles, identify coping strategies whilst fostering positivity and self-care.

Main duties of the job

Short term/medium term aims of the project
  • To incorporate a holistic care model supported by Allied health professionals to address the physical health need of this population.
  • To promote access to, health improvement resources available to service users.
  • Service users with a diagnosed mental illness are supported to make positive health changes using motivational approaches with the aim of :
  • Achieving and maintain a healthy weight
  • Eat a healthy balanced diet
  • Stop smoking
  • Be physically active
  • Drink alcohol within safe limits.
  • Maintain good mental wellbeing
  • Access screening and immunisation services in a timely way
  • Accessing community resources and services and being supported to engage in these.

The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.
