Community Psychiatric Nurse - (B5-B6 Development Role)

An exciting opportunity has arisen for a Band 5 Nurse to join the Mid Surrey Community Mental Health Team for Older People. This will be a developmental post, progressing to band 6 after 12 months, subject to evidence of competency, extending to 24 months to achieve.

We are looking for a highly motivated passionate nurse, to join the Community Mental Health Team Older People, who focuses on providing support and high quality nursing care for your clients and carers, targeting and prioritising people with severe mental health problems. You will manage a caseload of clients, and use evidence based/client centred principles to assess, plan, implement and evaluate interventions as part of the holistic approach. Excellent communication skills are essential, working alongside the MDT, working closely and collaborating effectively with all agencies and partner organisations involved in the care of our clients.

Main duties of the job

  • To act as key worker / care co-ordinator for named individuals, within the Care Programme Approach process, including Single Assessment
  • The ability to work alone
  • To provide high standards of care, which are delivered in a professional manner.
  • To ensure that the care delivered is based on best practice, and is regularly reviewed.
  • To ensure all records and reports are kept up-to-date, and up to the standard expected of this grade of nurse, in accordance with the Caldicott Report.
  • Keep an accurate, up-to-date diary and record of visits.
  • Be responsible for the continuous overall management of a defined caseload.
  • To receive and give clinical and management supervision / guidance
  • To supervise juniors / other staff in the delivery of care and treatment to individuals and groups
  • To support junior staff to manage caseloads effectively
  • To act as a positive role model for junior staff, assisting / guiding them in their duties
  • To provide information to the Line Manager, if any incidents that may affect the standing or running of the service
  • To develop close working relationships with voluntary / statutory services, in particular Social Services and members of the Primary Care Team, and participate in the system of integrated team working
  • To educate and train junior staff / student nurses and other professionals around competencies of a Community Mental Health Nurse
  • To participate in the Duty Rota to manage new referrals

Person Specification



  • RMN/BSC Hons or Diploma in mental health nursing
  • Membership of a professional body (NMC registration)
  • 1 years post qualification experience
  • Further training or experience to post graduate diploma level or 5 years equivalent experience


  • ENB 811, 812 or equivalent Community Nursing course



  • Experience in older people's community mental health nursing
  • Experience in care coordination and caseload management
  • Basic IT Skills including the ability to use a computer
