The Rapid Response Team is to provide 'hospital at home' to people during periods of acute illness and or Reablement in order to prevent hospital admission or to facilitate early discharge.The team works as a partner within a wider multi-disciplinary team.

The post holder will work collaboratively with and support members of the multidisciplinary team in the delivery, development and management of complex health and social care issues. In particular they will be responsible for ensuring the timely provision of a high standard of nursing assessment, advice, planning, implementation, training support and evaluation of care taking into account the social context in which the person lives. They will therefore be engaged in the planning and delivery of collaborative care packages that cross the health and social care boundaries.The post holder is expected to demonstrate and apply a broad range of nursing knowledge, interventions and skills. Knowledge of the principles of health promotion and preventing illness will be fundamental as will having a comprehensive understanding of the roles of others in health and social care.

To support the day to day operations of the team in the absence of the team leader. This is a unique opportunity to gain leadership and management skills as well as working clinically alongside a highly skilled clinical team.

Main duties of the job

  • To be responsible for the assessment, planning implementation and evaluation of care for community-based patients.
  • To work as a member of the wider primary care and community multi-disciplinary team
  • Administration of IV therapies within the community and Cedar Unit Clinic
  • To support the Team Leader, in developing the Rapid Response Team and raising the profile of the Rapid Response Service within the Borough

  • Registered Level 1 Nurse, Degree Level Education
  • Evidence of recent post registration, professional and portfolio development
  • Assessor/mentorship competency or willing to undertake training.
  • Clinical Patient Assessment


  • Post Graduate Qualification
  • Management Qualification



  • Significant experience working as a qualified RN
  • Assessment and provision of care to people with complex needs
  • Staff supervision/leading a team
  • Working as part of a multi disciplinary team
  • Caseload Management
  • Mentoring students and other health care professionals


  • Involvement of undertaking a training needs assessment and analysis
  • Staff caseload supervision

  • Able to perform extended clinical skills Intravenous therapy, cannulation, phlebotomy, syringe drivers, male and female catheterisation, removal of sutures
  • An understanding of the structure for the delivery for primary health care services.
  • Able to communicate succinctly and effectively verbally, in writing and electronically.
  • Able to present information effectively to individuals and groups


  • ECDL or equivalent
  • Counselling skills
  • Able to lead a team of nurses and support workers
  • Demonstrates ability to research, critique and change practices



  • Legislation affecting service area
  • Clinical Governance and how it affects practice
  • Knowledge and compliance with NMC Codes of Practice
  • An up to date working knowledge of :- Acute, chronic and terminal illness, wound management, care of the elderly, drug interactions, aetiology and physiology of pain, pain assessment, and strategies for effective pain and associated symptom control.
