Specialist Nurse / Paramedic - band 6 - Hertfordshire

We are looking for forward thinking, passionate nurse or paramedic who can take a positive risk management approach to support the frail population to remain at home when acutely unwell, within our Hospital at Home service in West Hertfordshire.

The Hospital at Home model has been developed with our system partners West Hertfordshire Teaching Hospital Trust and will blend digital monitoring and face to face care to facilitate acutely unwell patients out of hospital to home quicker or by preventing hospital admissions.

This service expands upon our existing virtual hospital models within our trust and has been implemented in line with National Guidance.

The team is led by a Frailty Nurse Consultant and the service delivered by Advanced Clinical Practitioners, Enhanced Clinical Practitioners, senior nurses, therapists, and support workers. The Hospital at Home service sits within the Unplanned Care Business Unit and aligns with CLCH's Urgent Community Response service covering the four localities in West Hertfordshire: Dacorum, Watford and Three Rivers, Hertsmere and St Albans and Harpenden.

Main duties of the job

System working with our acute hospitals will be a key aspect to the delivery of this service which will generate the opportunity for you to develop skills in system working and service development. Job description

Job responsibilities

To assess referrals made to the service, prioritise, and respond according to priority, as appropriate both in terms of level and urgency.

To use enhanced skills and knowledge to assess physical and psycho-social needs and develop a comprehensive and concordant plan of care with patient/carers and the wider Multidisciplinary Team as needed.

To make risk assessments, establishing the safety or otherwise of patients ability to stay at home, or return home safely from another care setting, providing, and arranging necessary supportive packages.

To provide evidenced based clinical/therapeutic interventions based on best practice, to improve health outcomes and promote choice.

To work in close collaboration with all other community agencies including staff working within the Community Adult Health Services.

Person Specification



  • NMC/HCPC registered with current valid registration
  • Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)


  • Experience of working in Community Services
  • Experience of assessing and providing quality care to people with Long Term Conditions
  • Qualification in teaching or practice-based learning.
  • Physical Clinical Assessment Skills Course or willingness to undertake the course.
