An opportunity has arisen to join the dynamic, innovative and award winning Frailty Team at University Hospitals Sussex NHS Foundation Trust as an Advanced Clinical Practitioner (Band 8a).

The role provides specialist assessment and management of Frailty patients in our brand new SDEC which is based in the fantastic new Louisa Martindale Building build overlooking the sea.

RSCH is an exciting place to work, our department having recently moved into the Louisa Martindale Building, we have plans to develop a co-located frailty SDEC from our existing Rapid Access Clinic for the Older Person (RACOP). Acute frailty services are also under development at Princess Royal Hospital. We are looking to develop our workforce to include Advance Clinical Practitioners (ACP) with expertise in the care of older adults with frailty starting with this role which will predominantly be based within our RACOP/Frailty SDEC team.

Main duties of the job

The role will be integral to the acute frailty team, primarily based across the two short stay acute frailty wards.

The core values of the acute frailty services are that all older adults with frailty accessing acute services should have:
  • Rapid identification
  • Comprehensive geriatric assessment at the point of contact by a dedicated team and
  • A clear plan for discharge from the point of contact.

    Job responsibilities

    The Advanced Clinical Practitioner will demonstrate highly developed specialist knowledge and skills in the management of care of frailty patients. Working autonomously or in collaboration with medical and nursing colleagues they will ensure patients are fully informed regarding their proposed treatment, provide appropriate information and support and liaise with relevant colleagues to ensure a seamless service for the patient.

    The Advanced Clinical Practitioner will formulate clinical decisions and complex treatment plans to manage frailty. The Advanced Clinical Practitioner will demonstrate highly developed specialist knowledge and skills in the management of care of frailty patients.

    They will carry out holistic needs assessments of patients, initiating supportive interventions and referring to members of the multidisciplinary team as appropriate. They will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care. They will be responsible for the development and review of care protocols and training for staff within their area of specialism.

    Person Specification

    Experience

    Essential

    • Professional Registration with either NMC, HCPC or GPhC.
    • MSc Advanced Practice or equivalent / other MSc mapped to the four pillars of advanced practice.
    • Able to demonstrate competence at an advanced practice level through Royal College or HEE Credentialing.
    • Evidence of Continuing Professional Development.
    • Registered with relevant regulatory body as an independent prescriber.
    • Physical Assessment and Clinical
    • Reasoning Adult/Child
    • Radiology Interpretation course
    • Teaching and assessing qualification.
    • Significant experience in specialty at band 7 or above.
    • Clear and demonstrable evidence of competent practice in the management of patients.
    • Evidence of multi professional, cross boundary working.
    • Evidence of audit/research and action outcomes
    • Demonstrate an up to date and detailed knowledge of relevant guidelines and clinical practice in Geriatric Medicine.
    • Evidence of recent teaching, mentorship, and supervision of staff.
    • Evidence of involvement in change management and/or clinical service improvement.
    • Evidence of theoretical and practical skills allowing autonomous patient management (i.e., patient assessment, diagnosis and management) related to clinical scope of practice.
    • Understand, encourage, and support clinical supervision and mentorship both within and across professional roles.
    • Effective leadership skills relevant to role (i.e., demonstrated ability to provide clinical professional leadership and mentorship)
    • Educational skills such as identification of learning needs, delivery, and evaluation, providing work based learning.
    • Experience of multi-professional working.
    • Ability to interpret and evaluate national and international guidelines and apply to local protocols and practice as appropriate.
    • Ability to communicate effectively with all stakeholders including to service users, clinicians, and senior leadership teams.
    • This will require proven ability and leadership skills demonstrating motivational, visionary, diplomacy and negotiation skills.
    • Works under own initiative with strong organisational skills and ability to achieve to deadlines.
    • Able to use high level problem solving and analytical skills and manage complexity.
    • Effective time management and prioritisation skills.
    • Appropriate level of I.T literacy (i.e., for radiology, pathology, microbiology ordering as per Trust / organisational protocols, utilising patient systems)
    • Experience teaching
    • Experience of Qi/Audit processes and reporting
    • Demonstration of experience working with older adults, people with frailty and holistic care
    • Ability to work autonomously, liaise and coordinate team and take appropriate action as necessary
    • Identify and adhere to best practice
    • Responsible for own professional actions and have sufficient autonomy for the delivery of the role
    • Able to interpret national and local guidance and to develop local policies to reflect these standards


    Desirable

    • Relevant NHS Leadership qualification
    • Leadership experience or qualification
    • Previous experience with geriatrics /frailty, CGA, SDEC
