An opportunity has arisen to join the dynamic, innovative and award winning Frailty Team at University Hospitals Sussex NHS Foundation Trust as an Advanced Clinical Practitioner (Band 8a).
The role provides specialist assessment and management of Frailty patients in our brand new SDEC which is based in the fantastic new Louisa Martindale Building build overlooking the sea.
RSCH is an exciting place to work, our department having recently moved into the Louisa Martindale Building, we have plans to develop a co-located frailty SDEC from our existing Rapid Access Clinic for the Older Person (RACOP). Acute frailty services are also under development at Princess Royal Hospital. We are looking to develop our workforce to include Advance Clinical Practitioners (ACP) with expertise in the care of older adults with frailty starting with this role which will predominantly be based within our RACOP/Frailty SDEC team.
Main duties of the job
The role will be integral to the acute frailty team, primarily based across the two short stay acute frailty wards.
The core values of the acute frailty services are that all older adults with frailty accessing acute services should have:
Rapid identification Comprehensive geriatric assessment at the point of contact by a dedicated team and A clear plan for discharge from the point of contact.

- Professional Registration with either NMC, HCPC or GPhC.
- MSc Advanced Practice or equivalent / other MSc mapped to the four pillars of advanced practice.
- Able to demonstrate competence at an advanced practice level through Royal College or HEE Credentialing.
- Evidence of Continuing Professional Development.
- Registered with relevant regulatory body as an independent prescriber.

- Significant experience in specialty at band 7 or above.
- Clear and demonstrable evidence of competent practice in the management of patients.
