We are seeking a dedicated Occupational therapist who is highly motivated and passionate about rehabilitation.
You will work alongside a friendly and dynamic multidisciplinary team to deliver high quality care and rehabilitation. You will use your skills and experience to provide individualised rehabilitation programmes to patients with a range of functional and rehabilitative needs, who are either recovering from an acute episode of care or step up from the community. Your focus will be on innovative rehabilitation programmes that enable safe discharge with ongoing plans being met through our community integrated enablement services.
You will have a understanding of patients with long term conditions, a significant frailty score and should be able to demonstrate a level of clinical assessment and reasoning, as well as being able to demonstrate knowledge and experience of rehabilitation through group work and individualised programmes.
You will be a highly motivated and passionate individual with excellent communication and organisational skills. You will need to be confident and assertive with excellent interpersonal skills and have the ability to demonstrate these in a pressurised environment.
We will consider staff looking for part time or job share opportunities. Access to a vehicle and possession of a full driving license is essential.

To provide a professional Occupational Therapy/Physiotherapy service to an allocated caseload in Community Therapy Team/Community Hospital Team. To assess clients, plan treatment / interventions and maintain associated records.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
