An exciting opportunity has arisen for an enthusiastic and experienced nurse to join a dynamic and forward thinking team. The successful candidate will work collaboratively within the Looked after Children's health team, and the Youth Justice service to assess health needs, and direct resources to meet the needs of the most vulnerable children and young people.

The role will require the nurse to:

provide high levels of nursing support/ intervention to vulnerable children and young people

have a keen interest and knowledge base regarding vulnerable children and young people.

have a keen interest and knowledge base regarding Youth Justice provision and secure settings.

be highly motivated, dynamic, adaptable and enthusiastic.

be committed to develop the current nursing service.

have at least 5 years post registration nursing experience, with 2 years being community based.

have excellent assessment, oral communication and written/IT skills.

have good organisational and time management skills.

work autonomously and as part of the wider support team around the child/young person.

Main duties of the job

The primary aim of the service is to achieve the best health and wellbeing outcomes through statutory health needs assessments, the healthy child programme, development/care reviews, immunisations, health promotion and interventions tailored to individual risks and protective factors, and to ensure that identified health needs are met and continue to be met when the young people are discharged from the service or transfer to a secure environment.

The post holder will work collaboratively within the Looked after Children's health team, the wider multi agency team within Youth Justice and will assess health needs and direct resources to meet the needs of these most vulnerable children and young people.

The role requires the nurse to deliver a broad public health provision. The focus of the role is both the delivery of evidence-based practice for young people presenting with a long-term condition, and the provision of preventative health care. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making.

Job description

Job responsibilities

To act as a lead professional, and plan, implement, and evaluate health intervention for vulnerable children and young people who are involved with Youth Justice Service.

The post holder will maintain a continuous process of reviewing, monitoring and evaluating care interventions to provide updated health information at key meetings and to other health professionals and multiagency professionals as appropriate.

Person Specification

Qualifications/Training

Essential

  • Registered Nurse, registration with NMC
  • Degree in Nursing relative to either: Children and Young People / Public Health / Specialist Community Public Health / Nursing, Health Visiting or School Nursing / Community Nursing Specialist qualification
  • Must be willing to participate in any relevant training identified to develop skills required to carry out duties
  • Maintains a portfolio of CPD in line with regulatory body standards


Desirable

  • Registered sick children's nurse
  • Nurse Prescribing. (Non medical)
  • Mentorship Qualification
  • Evidence of recent CPD including study in relevant fields
  • Post Graduate study relevant to the post


Experience

Essential

  • In order to fulfil the duties of this post, candidates should have attained a level of experience proportionate to the competencies detailed in the job description
  • Experience of working in a community setting
  • Significant recent practice with vulnerable and Looked after Children
  • Experience of delivering health promotion and public health frameworks
  • Able to deliver training to multi agency staff in a competent and professional manner
  • Experience of dealing with conflict and unpredictable environments


Desirable

  • Experience of working within a Multi Agency Team
  • Experience of delivering presentations, teaching sessions and training packages to individuals or groups
  • Experience of working with young people with mental health issues
  • Experience in supporting /signposting young people around sexual health issues
  • Experience of working with young people with learning difficulties

  • Specialist assessment, planning and evaluation skills
  • Detailed theoretical and clinical knowledge appropriate to service area and profession
  • Working knowledge of the Healthcare Standards for Children and Young People In Secure Settings ( 2013)
  • Working knowledge of the 'Knowledge, skills and competences of health care staff. Intercollegiate Role Framework (2015)
  • Working knowledge of the Promoting the health and welfare of Looked after Children Statutory guidance for local authorities, clinical commissioning groups, and NHS England (2015)
  • Knowledge of relevant current issues in health and social care, ethics and innovation.
  • Demonstrates awareness of limits to knowledge base
  • Knowledge of specific innovation techniques / initiatives.
