We can offer a full time Band 6 post working on the intermediate care wards in Tiverton Hospital supporting individuals during their inpatient stay and working with the wider MDT to arrange timely and effective discharges.

Tiverton Hospital has two Intermediate Care wards specialising in the rehabilitation of our Community residents following an acute Hospital admission. Our wards use an integrated team approach with our Nursing colleagues to support the patient to achieve their goals and maximise independence.
The Onward Care Clinician takes lead in admissions and discharges to and from Tiverton Community Hospital. They will work alongside the Multidisciplinary team to manage timely and effective discharge plans ensuring onward referrals are triaged appropriately and forwarded onto the correct teams. They will assist the Clinical Nurse Manager and Clinical Therapy Lead with day to day bed management of the ward.
We actively encourage professional development and have a regular in-house training and a supervision support system in place.
The successful candidate will need to have good IT and communication skills, and have the ability to work independently as well as part of the team.
We have a wide and varied caseload, working with patients with long term, Neurological and Orthopaedic conditions, as well as dementia, and advising on manual handling methods to support their rehabilitation needs and their transition home.
We encourage integrated working with our NHS colleagues in Multidisciplinary teams and also our DCC partners in the Social Care sector, third sector to deliver a high quality service in our locality.
The post holder will be responsible for the supervision and development of Band 5 Nurses/Therapist and unregistered Nursing and Therapy Support Workers.
We are committed to the continuing professional development of all our team members and offer a supportive environment with regular supervision and encourage on-going external learning as well as providing internal CPD groups, training and peer support.
Working Pattern: 37.5 hours per week, Monday to Friday 08:00 - 16:00
