We currently have a new vacancy within our growing team for a Children's Community/ Specialist School Nurse. For this position we are looking for somebody who is passionate about working with children and young people with complex needs and learning disabilities.

You will be part of a friendly and skilled community-based team providing a high-quality nursing service to children/young people with complex health needs across East Berkshire. This post is a dual role, providing specialist nursing support to pupils at Kennel Lane School (Bracknell) and children and young people with complex health needs in the community. You will provide a seamless and high-quality nursing service to Children/Young People and their families both in Kennel Lane School and within a wide range of community settings across East Berkshire. You will take overall responsibility for the assessment, provision, management, and evaluation of evidenced based nursing care to children and young people (CYP).

Your working pattern will be Monday to Friday, between the core hours of 08.00 - 17.00 (excluding bank holidays) but this may need to be flexible to cover short breaks training and end of life care.

Main duties of the job

  • You will support children/young people and their families to care for their child in their own home.
  • To work with parents/carers and other professionals to ensure that the assessed health needs of children and young people are met. This includes planning and delivering care and competency-based training to parents/carers, school staff and short break settings in conjunction with the Practice Development Nurse and wider nursing team.
  • You will identify and provide any equipment to support the treatment and care of the child or young person in the home and organise delivery of consumables.
  • You will plan and organise your own geographical locality caseload on a day-to-day basis, considering specific issues such as patient health needs, social and emotional circumstances, appropriate information sharing.
  • You will be part of the on call Rapid Response rota during working hours for unexpected child death across Berkshire.
  • You will be part of an on-call end of life rota (as required) in partnership with the local Children's Hospice to allow children and young people to remain in their home should they wish to.

The must haves for this role:
  • NMC Registration of a childrens qualification.
  • Excellent communication skills and the ability to demonstrate high level communication skills dealing with complex situations that arise within the school community and outside agencies.
  • Ability to use own initiative, manage own caseload and work independently.
  • Driving license and access to a car for work purposes.
