You will be an enthusiastic and experienced nurse, with a warm personality and great people skills. You will need to be adaptable to change and be able to influence new ways of working.

You will be a specialist resource for Gps and practice nurses, providing expert advice and support in order to upskill healthcare professionals in the management of people with both Type 1 and Type 2 diabetes.

You will be responsible for ensuring high quality evidence-based care for people with diabetes, their families, and carers.

Main duties of the job

We are looking for an experienced Diabetes Nurse as areas of service provision will include.
  • Support to General Practice with the management of people with diabetes.
  • Education and training to Healthcare Professionals involved in providing diabetes care in Primary Care
  • Provision of structured patient education for people with type 1 and type 2 diabetes
  • Support to community nursing teams, people with diabetes who are housebound, in residential and nursing homes and whilst inpatients on community wards.

Person Specification

Education/Qualifications/Training

Essential

  • Degree level qualification or equivalent
  • NMC Registration
  • Appropriate Post graduate qualifications in specialist area
  • IT skills/ qualification
  • Teaching certificate


Continuous Professional Development

Essential

  • Management/leadership training or skills
  • Evidence of a robust CPD portfolio - demonstrating specialist knowledge in the clinical field through attendance at appropriate courses and conferences


Desirable

  • Motivational interview training


Previous Experience

Essential

  • Relevant post graduate NHS Experience- at least 3 years
  • Experienced in the management of change.
  • Experienced in the provision of specialist treatments.
  • Responsibility for the management and appraisal of staff
  • Working independently as a team member
  • Working as part of a multi-disciplinary team


Additional Requirements

Essential

  • Ability to travel to multiple sites.
