An exciting opportunity has arisen for a highly motivated, caring and enthusiastic practitioner with a minimum of 3 years' experience at band 6 as an adult nurse, occupational therapist or a mental health nurse to join our dedicated Dementia and Delirium (DAD) Team. The post holder will have previous hands on experience of caring for patients with dementia and/ or delirium and supporting their carers. You will also feel confident delivering DAD related training. The post holder will also have a sound working knowledge of and experience of using objective cognitive assessements either in an inpatient or outpatient setting (such as Memory Clinic).

Main Duties and Responsibilities
Clinical Practice- Inpatients
1. To provide an expert specialist service for patients with complex needs and their carers
2. To provide expert advice and support to patients, their families and other healthcare professionals within the inpatient setting
3. To ensure continuity of a high standard of evidence based nursing care, assessing health, health related and nursing needs of patients, their families and other carers by identifying and initiating appropriate steps for effective care. This can include:
Managing a patient caseload
Making and receiving referrals
4. To accept responsibility for own patient caseload and help staff to ensure patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made ensuring clear documentation in the patients records and hand held records
5. To be actively involved in inpatient care, liaising with ward staff to ensure high quality care
6. To follow-up inpatients as appropriate and ensure all relevant information is available to enable patients to make informed choices about their treatment with the involvement of carers as desired
7. To work as part of a team to provide a seamless service through the development of enhanced Multidisciplinary team (MDT) processes and communication
8. To ensure all care is given in accordance with agreed protocols
9. To liaise with the ward staff to ensure appropriate and timely discharge/follow-up arrangement in place for patients with appropriate liaison with their carers
10. To liaise with local community, social services and voluntary organisations in order to access existing services for patients, carers and health-care professionals
Clinical Practice- Outpatients Memory Clinic
1. To work autonomously as a band 7 specialist in practitioner-led Memory Clinic
2. The DAD Specialist Practitioner will have continuing responsibility for assessment, monitoring and management of patients attending Memory Clinic and ensuring care is current, evidence based and in line with best practice standards
3. The DAD Specialist Practitioner will undertake objective cognitive assessments as appropriate
4. The DAD Specialist Practitioner will support family and carers and ensure they are sign-posted to appropriate community support services
5. The DAD Specialist Practitioner will refer on to community services to ensure patients and carers receive the optimal level of care and support
Education and Research
To provide information for clients, carers and heath professional about all aspects of specialist care
To ensure appropriate information and educational literature is available to help patients make informed choices as to their treatment plan and continuing management developing and regularly updating literature
To participate in the delivery and development of programmes of education to enhance the awareness, understanding and management of patients with dementia and delirium for all staff groups across the Trust.
To work with and support ward managers and other clinical staff to identify areas where changes to practice are required. Also where opportunities exist to improve treatment and support for patients
To actively participate in educational events provided by the Trust and primary care sector
To attend meetings and conferences as appropriate ensuring that you are fully conversant with current issues both within the Trust and within the specialist services locally and nationally
To coordinate clinical audit, in particular ensuring patient experience and carer surveys are completed and action plans developed
To take part in nursing research initiatives and support research activity within individual Divisions and also through collaboration with other agencies
To participate and assist with any Trust approved research projects conducted within the department
To publish and present relevant clinical practice and service developments.
3. Management
To play an integral role in the operational and strategic development of the DAD service
To work with the team to develop policies and protocols to improve patient care
To liaise effectively with all other disciplines within in the Trust and at local level
To participate in the management of change to improve standards and communication links from Kings College Hospital through to Primary Care
To actively contribute to any relevant initiatives within the Division and provide support to senior colleagues and Heads of Nursing
To act as an effective role model across the Trust
To work with and support managers and clinical staff to identify areas where changes in practice are required
To introduce measures to actively manage and reduce risk to patients. Ensure the formal reporting and recoding of adverse incidents in line with Trust policy.
To utilise and maintain information systems to aid audit and to provide regular actively analysis reports
To maintain records and send reports set by various bodies pertaining to the specific speciality
To maintain contemporaneous and accurate treatment records, submitting relevant statistics, reports and activity data as requested
To evaluate the service in terms of clinical effectiveness, clinical excellence and value for money.

- Registered General or Mental Health Nurse or Occupational Therapist
- Graduate level qualification
- Proven relevant post-registration clinical practice experience (Band 6)
- Understanding of models of management of dementia and delirium in practice
- Leadership experience or relevant qualification
- Able to demonstrate continuing professional development
- Can analyse and interpret information in an effective manner and exercise sound judgement as a consequence
- Significant and proven relevant clinical skills
