An opportunity has arisen for a band 6 Integrated Discharge Practitioner to work across the Bay Network supporting the mental health in-patient wards, ensuring timely and safe discharges. The post holder will support the ward teams to early plan for discharge with an emphasis on a holistic approach to ensuring the most appropriate post discharge support is in place.

We are seeking a dynamic and experienced mental health practitioner with a background in either social work, nursing or occupational therapy.

Main duties of the job

The post holder will join an established team of practitioners who are part of an exciting service looking to improve discharge arrangements for patients, particularly those with complex needs requiring funded packages of care.

This role will require the post holder to support clinical management and co-ordination of complex mental health discharge pathways, whilst working collaboratively within a wider multi-organisational team. You will work to developing processes as part of a co-produced service. It would be desirable for the post holder will require a working knowledge of commissioning processes, however in house training can be given.

In the role you will use your experience and knowledge of discharge/rehab pathways out of hospital provision as well as needing to be influential regarding discharge decisions options. You will promote alternatives to restricted environments whilst working with the patient and the clinical team to increase confidence in the combinatorial impact of multi-agency discharge support, balancing risk and responsibility.

The post holder will also have a role in reviewing the continuing needs of patients to ensure that they are progressing through their discharge plan to the least restrictive environment that will meet their needs for safety and service quality. The post holder will be responsible for making recommendations for patients who are suitable for stepdown to the community and/or lesser restrictive environment.

  • Relevant professional registration for example RMN, Social Worker or OT.
  • Educated to Degree Level in appropriate subject or equivalent
  • Several years post qualifying experience

  • In depth knowledge of mental health clinical practice and procedures.
  • Management skills
  • Expert skills in risk management, patient safety, prioritisation


Experience

Essential

  • Experience of operational management at Team Leader/ Deputy Team Leader/or clinical lead
  • Demonstrable experience of working in front line clinical service and understanding of pressures and ways of working.
  • Community Experience also essential
