We are looking to recruit an enthusiastic Band 5 Community Staff Nurse into our friendly and supportive team. You will contribute to the provision of high quality care, within a variety of community settings, which include the patients home and clinic settings. We believe in care that treats the whole person, not just the condition. We enable patients to stay at home longer and leave hospitals faster. Our community nurses work closely with members of the wider multi-disciplinary teams, including our GP's, local hospices, complex care nurses, health and social care coordinators, specialist nurses and therapist colleagues. Suitable if you are either a nurse looking for your first job in community nursing or are looking to continue or progress your community nursing career. Main duties of the job

The successful post holder will be required to work with a high degree of autonomy and professional accountability. You will be committed to working within a generic competency framework that enables you to have the clinical skills to work as a valued member of the team. The post holder will have responsibility for managing and co-ordinating their work, ensuring holistic assessments and care plans are recorded in line with trust and professional record keeping standards and always providing clinical care that meets acceptable standards of patient safety and quality outcomes. The successful candidate will be supported in their continued professional development and benefit from accessing a wealth of supporting services within the Trust. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
