We have an exciting opportunity for an experienced and innovative nurse to join the District Nursing Team within Solihull Community Nursing as an Out of Hours Nurse for 3 nights per week.
Applicants need to be self-motivated, dynamic and innovative with the ability to work autonomously. Previous experience of working within the community setting is essential.
Our teams are enthusiastic and motivated with a breadth of experience and a shared goal to offer the highest standards of quality care and good experiences to our patients. We work closely with other health and social care professionals across primary and secondary care, as well as with patients, families, and carers within their home setting.
The role involves providing the effective management of community patients to include patients with very complex needs, having responsibility for the setting of the highest agreed standards of quality and evidenced based care and the ability to embrace the challenges of the changing and growing demands of the primary care setting.
With any role within UHB, each post comes with the support of our Teams and Managers. This is extended by support form our School of Nursing, AHP's and Midwifery. Main duties of the job
* To provide effective routine management to the night nursing team resources and services.
* To promote and maintain standards of professional practices in order to enhance the quality of care to the patients.
* To provide effective leadership and develop the practice of the Night Nursing Service across Solihull Community Nursing.
* Have total responsibility for the assessment of patients with complex healthcare needs at night across the Trust and develop care plans to meet the complex health needs of patients against a range of community settings at night.
* To reassess patients with complex healthcare needs at night across the Trust, evaluate the effectiveness of the care provided and in meeting those needs and improve care.
* To hold responsibility for the management of the night nursing team as an entity.
You will need to be able to meet the needs of this service by being available to work flexibly between the hours of 21.00- 07.30 hours on a rota basis over a 7-day period and be able to drive.
