An exciting opportunity has arisen to work within the Specialist Palliative Care Team at South Tyneside Hospital.

This role is a developmental role to support a Band 6 nurse to develop into a Band 7 Specialist Palliative Care Nurse over a period of up to 2 years.

Opportunities will be given to support with training and education that is required for the role which would include Clinical Skills and Non-medical Nurse Prescribing.

A passion for working within Palliative Care and supporting with education and training to deliver a high quality service.

Main duties of the job

The job will be based within the Team at South Tyneside Hospital and will be to work flexibly mon fri 9-5.

Good communication skills and some knowledge and experience working with patients who have life limiting illness.

Experience of MDT working and managing own caseload. A desire to work within a Team who deliver high quality patient care and work with their extended team in the community.

Some understanding of the current local and strategic Palliative Care Issues and recommendations to support ongoing good practice.

Communicates effectively with patients where English is not the first language, those who have communication or language difficulties which could be due to cognitive impairment Communicates highly sensitive condition-related information to patients, relatives and professionals as required Ensures the maintenance of effective communication, written, verbal and electronic in multi-agency environments Within the Integrated Team: Clinical Team Manager acts as line manager and undertakes responsibility for operational and organisational requirements (sickness, performance management, allocation of resources, day to day management of the team) Specialist Practitioner works collaboratively as caseload holders and care coordinators, provides clinical leadership and support for the management of complex patients on the teams caseload Community Staff Nurse with the Specialist Practitioner is responsible for the work allocation, clinical leadership, support and guidance in the management of the teams activity

Person Specification

Qualifications

Essential

  • Current 1st level NMC registration
  • Degree in nursing studies or relevant clinical degree or evidence that currently working towards this (UK equivalent)
  • Mentorship certificate/Practice Assessor training or demonstrable equivalent experience
  • Clinical skills accreditation or willingness to complete


Desirable

  • Leadership and Management Course
  • Basic knowledge of financial management
  • Non-medical prescribing or willingness to complete


Physical Skills

Essential

  • Ability to assist patients to move as per Trust Moving and Handling Policy.
  • Carrying teaching equipment such as laptop, stationary etc.
  • Manual dexterity and up to date working knowledge of equipment e.g. syringe drivers, infusion pumps etc


Skills and Knowledge

Essential

  • Basic IT skills
  • Understanding of contemporary nursing issues
  • Evidence of continuous professional development
  • Knowledge of how to facilitate learning
  • Leadership skills
  • Knowledge of current professional and clinical governance issues
  • Knowledge of research and clinical audit

  • Experience of coordinating care for a caseload of patients
