If you are an experienced social worker, OT or mental health nurse, we would love you to join our team in the Early Intervention in Psychosis (EIP) Service - PATH (Psychosis: Prevention, Assessment and Treatment in Hertfordshire). Our service users are people experiencing psychosis for the first time and we are committed to delivering personalised and meaningful care.

The role will be working in the East and North of Hertfordshire in a specified geographical area, based in Stevenage.

Main duties of the job

As a Band 6 Care Co-ordinator, you will be:

• working together with your colleagues to provide our PATH service.
• cover a designated locality - not the whole county.
• travelling less and work out in the community, both in people's homes and our local hub buildings
• training as a family intervention therapist

You will be passionate in the delivery of recovery based and collaborative approaches to care planning, working flexibly and creatively with people in order to achieve their individual goals.

Our 'Caseload Mini Team' workforce model is unique and highly supportive of collaborative working. You will work as the Leader of the Mini Team, working closely with a Band 4 & 5 colleague to manage the caseload together.

• To plan and deliver Family Interventions for psychosis (FIp), in line with best practice.
• To plan and deliver Family & Friends Education & Support Group, in line with best practice
• To undertake comprehensive assessments and reviews, ensuring people meet eligibility criteria using social outcome assessment, and are assessed using the self-directed support Needs Outcome Assessment (NOA)

Essential

• Education to degree level or Academic qualifications equivalent (Diploma/Degree in in Mental Health Nursing /Social Work / Occupational Therapy)
• Registration with NMC/ HCPC/SWE

Desirable

• Advance Training in mental health
• Approved Mental Health Professional/Best Interest Assessment training

Experience and Knowledge

Essential

• Solid post registration experience inclusive of band 5 and 6
• Experience of working in acute mental health inpatient/day care services
• Experience of working in situations of crisis and emotionally demanding environments
• Experience and knowledge of safeguarding process dealing with family breakdown serious mental ill health and risk of selfharm.
• Experience of partnership working with service users and carers, demonstrating dignity and respect

Desirable

• Experience of teaching and training

Communication skills

Essential

• Excellent ability to communicate with a wide range of stakeholders including service users and carers, other professional colleagues, members of other organisations and local communities, verbally and in writing
• Experience of using electronic recording systems and ability to produce accurate, appropriate and timely reports.
