This post will involve key working families with neuro rehabilitation needs, the main focus being on coordinating their inpatient stay and discharge planning, supporting a transition into community services from the hospital. During a family's inpatient stay the post holder will be coordinating their rehabilitation with the rest of the IDT and ensuring the patients' complex rehabilitation needs are being met. The post holder will be expected to improve standards of care by supervising, teaching and supporting colleagues and family members. The post holder will also be expected to record patient outcomes after discharge, evaluate this data and make suggestions to improve the service where necessary. Once a patient has been discharged the post holder will be expected to continue support the family in the initial period until they have successfully transitioned into community health, social and education services . The post holder will also be expected to support consultants in outpatient clinic appointment and commence transition to adult services in a timely manner.
Main duties of the job
This is a fantastic and unique opportunity for an experienced Paediatric Neurosciences Nurse to join the neurology team working alongside consultants, allied healthcare professionals and other members of the interdisciplinary team. This post intends to improve the coordination of care children and families requiring neuro rehabilitation, following a diagnosis of a demyelination disease. The practitioner will work with the family, supporting their journey from critical or acute care through to community care, ensuring the health education and social care needs are considered throughout.
The successful applicant will be a key member of the MDT providing specialist advice to patients, families and other healthcare professionals. They will support and chair MDT meetings, participate in research and once the role has been established coordinate MDT clinics.
Discharge coordination, including close working with other agencies is integral to this role.
In return we will provide a comprehensive induction program, support further formal education and you will be appraised on a yearly basis with a view to developing knowledge and skills where appropriate.
