Solent are proud to offer a rehabilitation pathway dedicated at two separate inpatient sites. Both sites work as part of an integrated team across Adults Portsmouth delivering great care, and value for money. As part of an integrated approach to care, these units are led by a dynamic team of professionals who work closely with partners to join up care in the community, ensuring the best possible outcome for their patients and families by recognising diverse needs, treating people with respect, giving equal emphasis to physical and mental health

The inpatient facilities are situated across two sites, Summerlee in Fratton, is a 40 bedded unit supporting both Discharge to assess clients and rehabilitation. Spinnaker Ward based at St Mary's is a 16 bedded rehabilitation ward, aligned with our service line Adults Portsmouth. Our vision is to provide Quality Care which uses person-centred care while working closely with a wider multi-disciplinary team to achieve exceptional patient care.

This role will demonstrate excellent communication skills, using them to engage, persuade or negotiate with clients with complex, at times challenging needs to ensure they are provided with the most appropriate care and support.

Main duties of the job

*Proactively and positively contribute to the achievement of clinical outcomes through individual and team effort. Manage the patient experience and control risks.

*Support team members to deliver on their objectives through offering advice, guidance, and support as appropriate.

*Ensure that resources are used effectively and in accordance with agreed procedures

*Liaise with other healthcare professionals to ensure that work is neither overlooked nor duplicated

*Build and sustain effective communications with other staff involved in the shared services as required to ensure good team working and collaborative working practices

*Maintain and continuously improve in depth specialist knowledge relevant to the clinical area which significantly contributes to the Trust's stated objectives and aims.

*Increase the level of knowledge and skills within the Trust through sharing best practice and supporting others to develop their professional abilities.

*Dissemination of knowledge through report writing, and reviewing, taking full responsibility for clinical accuracy and reliability and being sensitive to the wider implications of that dissemination.

About us

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

Job responsibilities

*Undertake evidence-based practice for either developments relating to Trust work or opportunities for Trust involvement around health issues and contribute towards the continuing development of professional nursing practice.

*Ensure that expertise within your area is seen as a resource within and outside the Trust and form working partnerships with other health and social care service providers.

*Assist other clinical areas within the Trust if the clinical situation or staffing levels demand.

*To be primary nurse, supervising and managing delivery to a defined group of patients, ensuring that high standards of clinical care are maintained fully engaging with a multidisciplinary approach, where the contribution of registrants from nursing and the different health professions are recognised and deployed appropriately in the best interests of patient care.

*Be responsible for the assessment of patients' care needs and developing and implementing programmes of care/rehabilitation, including prioritising within a range of care/treatment options. Be responsible for the management of the patients records in your team, ensuring local standards are upheld.

*Ensure where appropriate that informed consent is obtained from patients for treatment and sharing of patient specific information in accordance with Trust procedure

*Rehabilitation and generic health assessments for patients with multiple pathology and complex needs. This will include:

oTissue viability

oDysphagia

oNutritional risk

oContinence assessment

oVital signs

oDischarge planning

oEnd of life care planning

*Use equipment and assessment tools to assist with accurate assessment of patients' health care needs and in determining the appropriate course of action, where necessary seeking advice from more experienced staff. E.g., falls assessment, nutritional assessment tool, and pain assessment

*Undertake clinical risk assessments under supervision and record the outcomes of such assessments using the appropriate documentation and standards as prescribed by the organisation.
