Senior Physiotherapist Intermediate Care Service, 37.5 hours per week
The role of the Intermediate Care Service is to provide efficient and effective patient focused care to all service users within primary and secondary care settings. This seamless provision ensures equity of service, patient choice and high standards of personalized care.
The role of the Intermediate Care Service is to provide specialised rehabilitation to patients within interim 24hr bedded settings, facilitating smooth transfer from Acute and Community Hospital settings. To assist patients to achieve their optimal level of independence and integrate back into their communities with the appropriate level of support for them and their carers. To prevent inappropriate hospital admissions and promote a supported discharge from hospital as early as is appropriate and safe. To provide high quality multi-disciplinary (MDT) assessment and treatment, advice/support and / or referral onto other agencies.
The referrals will predominantly be from both the Acute and Community Hospitals, but where appropriate from community settings where is has been recognised that a patient needs to step up into a service such as the Intermediate Care Service to prevent an admission to the Acute Trust.
Main duties of the job
The post holder will be required to work as part of the Intermediate Care Service providing the ethos of integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.
To practice in accordance with the Chartered Society of Physiotherapy Rules of Professional Conduct and The Health & Care Professions Council Regulations.
As a Physiotherapist in a multi-disciplinary team, work with other health and social care and 3rd Sector professionals to develop the ethos of multi-disciplinary, integrated care and seamless services ensuring that patient care and rehabilitation is delivered to the highest possible standard.
Working largely alone and unsupervised, to assess patients plan and implement rehabilitation and care plans as appropriate, in the intermediate care beds provide advice and education to patients, carers and colleagues and maintain associated records.

- CSP recognised Degree/Diploma or equivalent in Physiotherapy
- HCPC registered

- Evidence of attendance at relevant post graduate courses
