A new role has been created for a Community Mental Health nurse to work in the FACT service (Flexible Assertive Community Treatment)!
We are seeking to recruit an experienced nurse who is keen to work with service users who require immediate support to prevent escalation and admission to hospital.

Main duties of the job
The successful candidate will:
- be responsible for the clinical and operational day to day service delivery of care to service users who are receiving support from FACT
- undertake assessments of needs with recommendations around care planning and risk management
- undertake Connected Lives assessments including physical health assessments and assessments of medication side-effects
- participate in Multi Disciplinary Team working
- supervise Band 5 Nurses, Nursing Associates, Health Care Support Workers and Support Time and Recovery Workers and students.

- To maintain effective joint working with others, developing positive working relationships and clear communications with the FACT team, MDT and other professionals.
- Ensure that all relevant information regarding service users care in particular issues of concern regarding practice and workload are communicated and documented as necessary to the FACT manager.
