The Community Therapy Team model aspires to provide a responsive, safe effective caring service that is well led. The team are looking for a passionate physiotherapist to join the friendly, cohesive team of therapists and therapy assistants, to provide domiciliary rehabilitation to the local community in Mid Hampshire. You will develop your holistic treatment skills and expertise in treating a wide variety of healthcare conditions and enhance your network of healthcare professionals. You will also have the opportunity to develop your leadership and management skills.

Main duties of the job
This post's primary role is to provide strong holistic care to the community, encompassing a variety of clinical conditions covering all aspects of physiotherapy. You will use transferable skills from previous experience to manage own case load, identifying the needs of the patients, set patient centred goals and treatment plans using evidence-based practice promoting functional independence and preventing unnecessary admission to the acute hospital. You will be required to use effective communication skills to triage, delegate to rehab assistance and mange onward care, working effectively within a multi-disciplinary team and wider healthcare colleagues.

This post will have the opportunities to be innovative and develop the service and themselves with access to CPD in house training and external course support. It is essential you possess a current driving license with business cover and access to a car.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
