We have an exciting opportunity for a Band 6 Nurse to join our Special Needs School Nursing Service.

The job entails working with children and young people with complex health needs in local Special Needs Schools.

You will be part of a vibrant nursing team who are committed to providing the best care possible for children and young people with SEND.

Main duties of the job

The Special Needs Nurse will have responsibility for the day-to-day management of an agreed caseload, support with the development of the service and provide leadership to junior members of the special needs school nursing (SNSN), team.

To be a source of support, information and advice for children and young people (CYP) and their families, accessing special needs education settings. Liaising with key partners to support CYP with complex health needs and learning disabilities.

The post holder will be required to work in partnership with CYP with complex needs, their families, and the multi-disciplinary team to enable the child to attain the optimum life experience.

The post holder will be a member of the Special Needs School Nursing Team, working with CYP with additional needs to support high quality person-centred health care. They will have the ability to work independently under their own initiative with the support of the team manager.

To participate in the delivery of health promotion through individualised health assessments completed with the CYP.

The team also advise, support, and provide training to special school staff to identify health needs and implement a child's individual health care plan, enabling, CYP with health conditions to access education.

Job description

Job responsibilities

To have oversite of and lead on the delivery of the service to a defined caseload of CYP.

Alongside the team lead, to act as a link between health, education, and other members of the multidisciplinary team, to ensure that the health needs of the CYP on the caseload are being effectively managed, enabling access to education, within special needs school settings.

To delegate tasks as appropriate to other members of the SNSN team and provide leadership support, to enable them to carry out their role effectively.

Work collaboratively with the wider multidisciplinary team, in-line with relevant legislation and guidelines, to support those attending special schools to improve health outcomes, ensuring that identified risks are escalated and managed within agreed pathways, such as safeguarding.

To be instrumental in adapting the service, to meet the changing needs of service users, working within agreed frameworks of care and organisational guidelines.

To work collaboratively with schools to identify health improvement initiatives and support the delivery of these within special schools, reviewing the effectiveness of any intervention.

Participate in the assessment of health need, implementation, and evaluation of specialist health care plans to meet the needs of the individual CYP to enable them to remain in education.

Provide health education information to the CYP and family/carers/education staff, to promote a healthy lifestyle.

Advise and support education staff to implement clinical care plans.

Provide a package of training to enable education staff to support CYP with specific conditions to attend school.

To perform clinical nursing procedures as required and to provide support to educational staff to enable them to respond appropriately to emergency situations.

To facilitate onward referrals to other services as required.

Undertake ongoing caseload reviews.

To participate in health and medication reviews as an advocate for the CYP and family to ensure the health care plan considers and supports access to day-to-day activities.

To facilitate transition of CYP to adult services, following an agreed pathway.

Person Specification

Qualifications and Training

Essential

  • Evidence of continuous registration on the Nursing Midwifery Council Register
  • RSCN, RN Child Branch, Learning Disability Nurse or RGN with transferable skills
  • Experience of supervising/line management.


Desirable

  • Practice assessor qualification
  • Clinical/physical assessment skills course
  • Leadership course


Experience

Essential

  • Evidence of ability to plan care to meet the needs of the CYP.
  • Experience of teaching others
  • Evidence of Professional Development
  • Ability to work independently, under own initiative to achieve set tasks
  • Evidence of relevant post registration experience including multi-disciplinary working.

  • Quality/Audit initiatives
  • Knowledge and experience of supporting children with special needs.
  • Knowledge of the SEND Agenda.
