- Work as part of a multi-disciplinary team in supporting the CMHT members in the delivery of a high quality community mental health service for older people with mental health needs.
- To work under the supervision of the registered team member and in conjunction with health care assistants and colleagues from the local authority.
- To participate in the delivery of a service for those people under the care of older persons memory services within the CMHT.
- To maintain a professional responsibility whilst providing support to individual and their care partner's known to Memory services within Older Peoples Mental Health services.
- To establish a rapport, and maintain supportive professional relationships with clients and their carers, even when there are barriers to acceptance such as lack of insight or difficulties adjusting to complex life changes recognising the stress associated with caring, offering and undertaking carers assessments as required.
Main duties of the job
To work collaboratively with other relevant agencies and staff groups to ensure effective team and partnership working, coordination multi agency activities when appropriate.
Contribute to the Implementation of NICE Guidelines, National Service Framework where applicable.
To help supervise Health Care Assistants in the delivery of quality patient care.
To attend multi-disciplinary meetings where appropriate ensuring good communication with all team members.
To participate in case supervision with the Team Manager.
To ensure that risk assessments have been undertaken on all clients by the care coordinator, ensuring at all times the safety of you and other team members.
To accommodate students on placement in the team promoting a positive learning environment.
Actively seek opportunities to continually develop oneself, consistent with the principles of lifelong learning and in line with the professional body's continuous professional development requirements.
To keep updated in clinical developments and to ensure evidence based practice is delivered to patients/clients referred to the service.
Ensure the safe administration of oral/intramuscular medication as prescribed. Monitoring therapeutic effects and side effects, liaising with the appropriate staff regarding the physical and mental state of the patient.
The ability to speak Welsh is desirable for this role Welsh and/or English speakers are equally welcome to apply.
