We are looking for Occupational Therapists to join the South Cumbria Community Therapy Team, based from the community offices at Fairfield Lane, Barrow in Furness. As part of the team, you will support the delivery of high quality Occupational Therapy interventions to patients in their own homes and our new intermediate care bed facility. Therefore, you must have the ability to work and travel to various sites across the trust independent of public transport. The team prides itself on agile working with team members spending time working from home, local clinics and the team bases.

The service works closely with the Integrated Care Communities, local GP's, Adult Social Care, and hospital discharge teams.The service supports people with a wide variety of conditions including elderly care, frailty and falls.

This is an exciting time to join the team as we support the development of our new intermediate care beds and continue to expand and evolve our services to meet the needs of our community.This is a fixed term vacancy or a secondment opportunity.The posts are part -time, however full time hours will be considered by combining available hours.

Main duties of the job

To work as an individual within the community therapy/intermediate care team, undertaking specialist occupational focussed assessments and treatment of patients with frequently complex problems in their own homes, community hospital or residential/nursing home/intermediate care beds.

To work as an autonomous practitioner taking responsibility for the management of patients on own caseload without direct supervision.

To continue to develop specialist clinical skills, knowledge and experience through participation in In-Service training and specialist Personal Development as identified in Personal Development Plans, (E.g. specialist external courses, shadowing etc).

To maintain effective communication links with patients, carers and all associated professional practitioners, including the completion of accurate clinical records in line with trust and professional body codes of practice.

To regularly assist in the planning & delivery of team and departmental training and education sessions, to further professional and personal development for staff, working with the guidance and support of senior members of staff if required

To participate in research and audit as requested by the team lead/clinical leads

To be responsible for the clinical education and practice of Pre Registration Occupational Therapy Students accepted on clinical placement.

The team delivers a 5 day service Monday to Friday within the core service hours of 0830 to 1630

We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.

You will be expected to be able to travel across south Cumbria and be flexible for the needs of the service with occasional expectation to work across traditional boundaries. A broad experience of delivering rehabilitation is advantageous.

In addition to a comprehensive induction package we offer Team support, staff supervision and in-service training opportunities.

Person Specification

Education and Qualification

Essential

  • Health and Care Professions Council (HCPC) registered Occupational Therapists
  • Degree in Occupational Therapy approved by the Royal College of Occupational Therapy
  • Post graduate training or work experience to post graduate diploma level 7 or equivalent
  • Ability to Independently Travel between trust sites and to places of residence not covered by public transport.


Desirable

  • Experience of trial of concept or audit process
  • Membership of the British Association of Occupational Therapists.
  • Practice placement educator accreditation/Experience
  • Experience/Qualification in assessing for and prescribing moving and handling equipment and advice (training can be provided)


Experience

Essential

  • Post registration experience as an Occupational Therapist to achieve knowledge and skills transferable to this post
  • Experience of supporting un-registered support staff/and or Students
  • Proven ability to manage a case load effectively
  • Ability to contribute too and where appropriate lead case conferences and patient related meetings
  • Ability to use technology and IT as required for the post (Microsoft office, electronic patient records etc)
  • Knowledge of risk assessments tools and positive risk management


Desirable

  • Experience within Community or Rehabilitation setting
  • Experience working with older adults/people with frailty syndrome.
  • Experience prescribing aids and adaptations


Skills, ability and knowledge

Essential

  • Transferable knowledge and skills at basic practitioner level with aptitude to develop to specialist OT practitioner level
  • Ability to develop effective working relationships / team working
  • Ability to analyse professional and ethical issues
  • Ability to work under pressure and recognise when support is required
  • Ability to reflect and critically appraise own performance.
  • Applied knowledge of health and social care legislation and guidelines e.g. National service Frameworks (NSF's) and National Institute of Clinical Excellence (NICE) guidelines for clinical conditions and application to OT practice.
  • Written and oral communication skills in English sufficient to communicate effectively with patients to gain consent and explain treatment interventions and gain information required for the patient's medical/social history relevant to Occupational Therapy
  • Basic computer skills e.g. word processing, email and internet skills.
