An opportunity has arisen for an experienced cardiac nurse to join the community cardiac nursing team .

The post holder will be required to work within their area of expertise, utilising their expert level of cardiac knowledge and enhanced clinical skills to assess and support the patient. You will work autonomously providing effective evidenced based care and you will be expected to manage a caseload of complex patients, that includes those with heart failure.

The post holder will be based at St Austell Community Hospital and work within a designated area.

To provide clinical assessment and specialist nursing care without direct supervision. Ensuring that every patient is assessed and his/her care is planned, implemented and evaluated, and that this is documented.To manage a case load of the more complex cardiac patients, this includes mainly heart failure patients. Including the management of drug therapy plans and monitoring therapy in liaison with GP's. The job holder will need to have an expert level of cardiac knowledge and enhanced clinical skills to assess and support the complex patient.

To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures and guidelines.

Main duties of the job

You will be ensuring that the care that is planned and delivered is evidence based and in line with local and National policies and guidelines.
  • To independently manage a caseload of complex cardiac patients. Delivering care within the home setting or as clinic based or group activity or as telephone support,
  • To ensure patients referred to the service are contacted within the agreed time scales.
  • To use clinical assessment skills including physical examination, detailed history taking, diagnostic skills and treatment planning to support the care of individual patients.
  • To use advanced levels of clinical decision making through specialist assessment and interpretation of clinical information to support patients and carers who have complex and/or rapidly changing physical symptoms, emotional or practical needs
  • To improve the management of heart failure patients by optimising treatment and care in line with evidence based guidance.
  • To manage a case load of the more complex cardiac patients, this includes mainly heart failure patients. Including the management of drug therapy management plans and liaise with GPs and consultants.

Qualifications

Essential

  • Post reg experience within cardiology
  • Current professional registration - NMC adult
  • A relevant post registration qualification - eg: ENB 124, 249 or 254.
  • Working towards a degree
  • Mentorship module
  • Evidence of recent relevant personal professional development
  • Hold a post-registration diploma


Desirable

  • Degree level Heart Failure course eg: Caledonian and/or post graduate qualification in rehabilitation
  • Extended and Supplementary Prescribing Qualification


Experience

Essential

  • Ability to work autonomously and caseload management ability
  • Evidence of managing a caseload
