The Rotherham NHS Foundation Trust Tuberculosis (TB) Nursing service is looking for a dynamic person to work as a Band 6 Tuberculosis specialist Nurse. We are looking for an experienced and enthusiastic, forward thinking nurse for a diverse role in an established community TB service. You will need excellent communication skills and enjoy using your initiative and providing support to colleagues within the team. Full training and support will be provided for the right candidate. The post holder will work in the community based TB service and will provide care across the Rotherham community predominantly working in patients homes. You will enjoy working with adults, young people and children from diverse communities and backgrounds, this includes those deemed to be from an underserved population. You will provide outpatient clinics, both in the community and hospital setting, working closely with other professionals as part of a multidisciplinary team. Care delivered will include supervision of treatment including Video/ directly observed therapy. The TB service focus is about public health and the post holder will maintain an excellent knowledge of the physical aspects of the disease, the emotional, psychological and social impact on individuals, their families and friends. The role will involve contact tracing and screening and to deliver any associated interventions required to support to the wider population.
Main duties of the job
Manage a caseload and providing standard/enhanced case management (ECM) to patients (both adults and children) affected by active TB disease or latent infection. Plan, deliver and evaluate interventions and/or treatments for all forms of TB and LTBI, when there are complex issues and any adverse drug effects. Contact tracing Plan and risk assess community situations to include cluster investigation and large contact tracing exercises. Communicate with people from all cultures, backgrounds, frequent use of an interpreter is required. Order the appropriate tests required to diagnose and monitor patients receiving treatment for drug sensitive and drug-resistant TB. Record activities and communications consistent with legislation, policies and procedures Undertake personal development and contribute to the development of others by training colleagues in direct contact with those affected by TB. Attend and where appropriate, contribute to meetings, forums and case conferences. Provide expert knowledge to other professionals, agencies and identifies self as patient advocate Promote, monitor and maintain best practice in health, safety and security, in collaboration with the infection control team. Triage all referrals and autonomously plan, co-ordinate and monitor care for patients requiring ECM. manage and support joint T B clinics and nurse led clinics.
