An exciting opportunity has arisen for highly motivated individual to join our Community Nursing Team covering Central Shropshire in the provision of Planned and Admission Avoidance care to patients in the community.

We are looking for a caring, motivated and friendly community nurse to join our busy, professional and dynamic team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care. You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development.

Main duties of the job

Promotes a 'can do' attitude and determination to develop and sustain a dynamic and responsive administrative and operational service within a community setting which delivers best practice.

Able to assess, plan, implement and evaluate interventions, liaising withcolleagues and partners to ensure an integrated approach to the needs of the service user.

To organise and deliver skilled clinical support work without direct supervision. This will include individual in a variety of settings.

The team provides nursing care to patients within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.

We are committed to clinical support, CPD & training.

The post is covering 7 days 8am to 6pm service, on a Rota Service.
