We have an exciting opportunity for an individual (Registered Nurse) to join a successful and established Continuing Healthcare (CHC) team on the Wirral. The team is responsible for providing commissioning services for people with complex care needs who are eligible for NHS funded care.
This is a permanent post, working Mon - Fri and is an exciting hybrid of patient contact and commissioning care on an individual basis.
The post offers superb opportunities to utilise your assessment and case management skills whilst learning within a supportive, experienced and successful team. The candidate should have experience of undertaking CHC assessments, working with patients, families and carers, and have a sound knowledge of the NHS Framework for Continuing Healthcare. The successful candidate will be responsible for the process from referral to assessment, review, care commissioning and case management for individuals eligible for CHC
You will need to be an Registered Nurse with current registration and be able to work independently and as part of a team, be comfortable using initiative and knowing when to ask for guidance. In return you'll have access to all necessary support, administration help and clinical supervision. This role ordinarily involves travelling across the Wirral with occasional visits further afield therefore a valid driving licence and access to a vehicle for work purposes is essential.

- Ensure assessments are undertaken in line with the National Framework for Continuing Healthcare and funded-Nursing Care and submitted to the CCG within the service's key performance indicators
- Complete all review assessments in line with the service's key performance indicators
- Work closely with acute hospitals to support discharge to assess pathways
- Accurately input information onto departmental databases and tools used in Continuing Healthcare adhering to NMC standards for record keeping
- Assist and support individuals and representatives in relation to the Continuing Healthcare process
- Commission appropriate packages of care for individuals eligible for CHC or other NHS funded care, in line with the Commissioning Policy.
- Case management and ongoing review of individuals eligible for CHC or other NHS funded care in accordance with the National Framework for Continuing Healthcare and NHS-Funded Nursing Care
- Provide assurance regarding quality of individual packages of care by undertaking reviews as required and reporting quality/safeguarding concerns through the agreed reporting route in a timely manner
- Undertake urgent reviews of individuals where quality and safety concerns have been identified.
