We are searching for a Personalised Care Project Lead with significant experience of working within the Cancer Nursing field. The post is a fixed term post for 12 months as a 1WTE working collaboratively between two sites. Two days will be onsite at East Cheshire Trust and two days will be onsite at Mid Cheshire Hospitals NHS Foundation Trust.
The successful applicant must have excellent communication skills and work well as part of a small team. Previous experience of working with cancer patients or within a cancer setting is essential.
East Cheshire and Mid Cheshire Hospitals NHS Foundation Trust are working in collaboration with Cheshire and Merseyside Cancer Alliance (CMCA) to ensure that all patients with cancer diagnoses have access to personalised care interventions Personalised Care and Support Planning is a core deliverable of the NHS Cancer Plan and ensures that patients and their families are supported to live with and beyond their cancer diagnosis.
Hours per week: 30 hours
Main duties of the job
The aim of this project is to ensure that Personalised Care and Support Plans (PCSP) (based on Holistic Needs Assessments (HNAs)) are available for all cancer patients, and data is submitted to Cancer Outcomes and Services Data set (COSD).
Personalised care has been identified as a key priority within the NHS Long Term Plan and the aim contributes to the overall Long-Term Plan personalised care ambition that: Where appropriate every person diagnosed with cancer will have access to personalised care, including needs assessment, a care plan and health and wellbeing information and support.
This project is for a period of 12 months ensuring that new and existing processes are embedded within clinical teams and sustainable at the end of the project.
The post holder will lead the development, coordination and implementation of the personalised care agenda, the community and work with primary care to increase awareness of signs and symptoms of secondary disease. The project will also support teams with patient stratified follow up ensuring all patients are given quality information in end of treatment summaries.
