The cohort of children and young people we are looking to support with the new CNS roles, are under the care of several medical specialties. Their medical conditions are life-long and complex, and the pressure placed on their parents/carers is relentless.

The complexities and health needs of these CYP can result in fragmented, inconsistent, and uncoordinated care which often causes confusion and unsatisfactory outcomes for them and their families.

Roald Dahl CMC Nurse Specialist will enable proactive and inclusive services centred on the needs of the child and family. The benefits would include reduced emergency admissions, shorter hospital stays, improved communication, improved resilience, enhanced relationships between families and professionals, and greater information sharing across organisational boundaries.

Main duties of the job

To provide clear and effective leadership and Co-ordination of the patient pathway for children and young people (CYP)with medical complexity (CMC)

To provide a high-quality service meeting the individual needs of the CYP and their families.

To work collaboratively with all stakeholders including MDTs, CNSs, clinical teams, Consultants, therapists, and community nursing teams across the ICB to facilitate improved patient flow, capacity and continuity of care.

To be an expert in the delivery of clinical skills and knowledge in CMC to promote good practice.

Effectively liaise with education and social care to reduce fragmentation of care across organisational boundaries.

Manage a case load of CMC working autonomously while consulting and escalating concerns as needed

Provide evidence-based teaching/training to CYP and their families, providing up to date evidence based verbal and written information/leaflets about their conditions that is age and culturally appropriate to promote and develop their confidence in self-care.

Leading agreed projects related to practice and professional development.

Use expert knowledge to initiate the process of discharge planning at the earliest realistic point.

Use expert knowledge to identify and work with joint agencies within the community to carry out individual needs assessments to identify the level of support the family will require at home.

Use expert knowledge and experience to assess, plan, implement and evaluate the care pathways for children and their families presenting for care within the framework of the nursing team.

Lead in developing pathways of care to improve the quality of the patient experience.

Use expert knowledge to develop continuing care and discharge planning focused on the individual patient maintaining a dynamic researched based approach and the development of clinical practice undertaking regular reviews in conjunction with the multidisciplinary team.

Use expert knowledge and experience to work as a member of the health care team in the delivery of quality care and support others appropriately.

Use expert knowledge and experience to act as role model to promote best practice.

To lead in the provision of individualised care pathways to the child and their family requiring continuing care within the framework of team nursing.

Use expert knowledge and experience to participate in meetings about the ongoing psychological, social, and developmental needs of the individual child/adolescence.

Lead in the introduction and use of electronic passports for CMC.

Qualifications and Training

Essential

• NMC child Registered Nurse (RN child/ RSCN)
• First Degree in relevant subject or equivalent qualification/experience
• Teaching/mentorship qualification
• Evidence of continued professional development and training

• Management/leadership qualification

• Excellent communication skills, both written & verbal
• Ability to plan, organise and prioritise own work
