We have an exciting opportunity for someone with the right experience to join the expanding Notts HC Patient Safety Team. Role is in improving and embedding a safety culture in the journey of an improving organisation who are focused on providing the best care to those who use or need services .

The job purpose for the Patient Safety Investigator /Reviewer is to facilitate and complete a range of learning responses to patient safety incidents and contribute to Patient Safety Incident Investigations (PSII) within the Mental Health, Forensic and Community Health Services Care Groups. These findings are then used to identify effective, sustainable improvements by combining learning across multiple patient safety incident investigations and other responses into a similar incident types. Integral to the post is working directly with families, carers and patients affected by patient safety incidents.

The post holder will use their professional and clinical knowledge and experience to provide expert risk management, patient safety advice and support to healthcare professionals and managers throughout the investigation process to ensure compliance with safety, quality risk and effectiveness standards

The Investigator will work in line with Being Open and Duty of Candour principles, ensuring appropriate investigation/review methodology and systems-based approach to support the Trust's patient safety improvement work.

Main duties of the job

In line with PSIRF, the post-holder will work within the Patient Safety Team as a PSIRF Investigator/Reviewer to facilitate and complete a range of learning responses to patient safety incidents and contribute to Patient Safety Incident Investigations (PSII) within the Mental Health, Forensic and Community Health Services Care Groups.

The investigator role will also support to facilitate and undertake robust learning responses and investigations and to meet the required deadlines for submission to external stakeholders.

The post holder will have their own caseload of investigations which they will lead and complete to a high standard. At times, due to clinical expertise, the postholder will be required to lead or contribute to reviews within other care groups.

The post holder will use their professional and clinical knowledge and experience to provide expert risk management, patient safety advice and support to healthcare professionals and managers throughout the investigation process to ensure compliance with safety, quality risk and effectiveness standards

The post holder will continue to maintain the communication and liaison with management within the services as required for their role.

The post holder will be required to work directly with families, carers and service users

Job description

Job responsibilities

The post holder will take responsibility for identified learning responses and actions to patient safety incidents. Generating investigation findings and recommendations which improve patient safety.

Work to the NHSE Patient Safety Incident Response Framework (PSIRF) standards.

Use a systems based approach when undertaking reviews and investigations of patient safety incidents.

To enhance and promote quality improvement of services provided through evidence based practice, ensuring that systems and processes are in place to support this whilst incorporating best practice and national service guidelines.

Understand the complexities of mental health practice and services.

Produce timely reports of high quality that patients, families, carers and staff value, trust and respect.

Signpost those who need further support post incident appropriately including families, carers significant others and staff.

Escalate any emerging and urgent clinical risk patient safety issues appropriately.

Adhere to strict confidentiality and has the ability to prioritise and work on own initiative.

Liaison with professional leads to ensure specialist advice and knowledge is sought during the process of any investigation. Communicate with carers and significant others throughout the investigation process.

Person Specification

Qualifications

Essential

  • Educated to Degree level or equivalent (or relevant work experience)
  • Evidence of continued professional development
  • To hold a registered clinical qualification


Contractural Requirements

Essential

  • Ability to work to meet the needs of the services
  • A full UK driving license and vehicle for business use is required for this post however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
  • Ability to travel across all Trust sites over a wide geographical area


Physical Requirements

  • Requirement for sitting in a restricted position for a considerable part of the working day
  • Requirement for travelling to other sites a few times a week

  • PSII/ SEIPs investigation training or willing to work towards.
  • Level 1 and Level 2 patient safety syllabus


Experience

  • Experience of working at a senior level within a quality, risk management and patient safety incident investigation role.
  • Experience and understanding of the NHS Patient Safety Incident Response Framework.
  • Experience of undertaking system investigations of patient safety incidents.
  • Clinical Experience of working within mental health, forensic and or community settings
  • Extensive experience in report writing.
  • Experience of facilitating significant event reviews.
  • Experience of delivering training.
  • Experience of presentation skills using a variety of media/styles.
  • Experience of engaging with services


Desirable

  • Experience in the training of staff
  • Relevant experience working within patient safety
  • Experience of working with IT systems.
