Physiotherapist, Occupational Therapist or Nurse Assessor
There is an exciting opportunity for a Physiotherapist, Occupational Therapist or Nurse Assessor to join the Hospital Intervention Team (HIT) at East Sussex Healthcare. There are opportunities for full and part time posts and flexible working within service hours. This is a secondment opportunity covering maternity leave for 1 year.The team cover a 7 day service 8am-8pm across AandE and gateway wards to the hospital . HIT are a multidisciplinary team consisting of nurses, OTs and physiotherapists. They work to facilitate hospital discharges and prevent hospital admissions. The team have opportunities to work flexibility and also go out into the community to see patients in their own homes as needed.
Main duties of the job
The successful applicant will join a highly skilled multi-disciplinary team who work within the gateway wards of the acute hospital. The teams are tasked with reducing unnecessary admissions, and facilitating safe and timely discharge to patients who do not require in-patient stays. To deliver this essential and high profile service you will require high level clinical reasoning skills and the ability to problem solve under pressure. Experience within the core areas of the profession is essential, as the caseload is variable, with many patients requiring multi-modality interventions.The post holder will be integral in developing the inter-disciplinary model of assessment and treatment, and in achieving key standards of practice within East Sussex Healthcare NHS Trust. There may be options to rotate within other clinical areas in the future if requested. All applications will be scored against the Person Specification and so applications should reference to the key criteria of this role. Only applicants with current HCPC or NMC registration will be considered for shortlisting.

Qualifications
Essential
- BSc Physiotherapy/ Occupational Therapy/ Nurse MSc in Rehabilitation or equivalent
- Current HCPC/NMC registration
- Evidence of continued professional development to diploma level or equivalent level of knowledge and experience
Desirable
- ICES Trusted Assessor's certificate
- Post graduate qualification in relevant area
- IT certificate (e.g.ECDL)
Experience
Essential
- At least 2 years as a band 5 Physiotherapist/Occupational Therapist/Nurse
- Recent experience in a hospital or community setting
- Experience working within Gateway wards, or in discharge/ admission avoidance teams
