An opportunity has arisen for a developmental B6 post working alongside the Stroke Specialist Nurses. The post will provide insight into managing patients presenting with suspected stroke, and develop key skills required in order to perform this role effectively. This is a developmental post with the aim to work independently as a B7 Stroke Specialist Nurse once assessed fully competent. The successful candidate will be required to undertake further training in the clinical assessment of patients, and the non-medical prescribers course at some point.

The Stroke service within Derriford Hospital is expanding and this is an exciting time to join the team, and to help take this service forward

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Main duties of the job

1. Assist with investigations for patients with suspected stroke attending the Emergency Department

2. Clinical duties such as, but not limited to, administration of antithrombotic medication preparation for mechanical thrombectomy swallow screening and nutrition management fluid management including intravenous fluid management continence management including intermittent catheterisation and insertion of indwelling catheters cognitive screening cannulation and venepuncture

3. Liaise with the ASU and the bed management team to ensure that all new patients reach the stroke unit within 4 hours of hospital arrival

4. Where necessary help transport new admissions to the Acute Stroke Unit

5. Communicate effectively with various other disciplines involved in the stroke pathway

6. Help collect SSNAP data accurately

7. Maintain and update the library of stroke information leaflets at ward level and link with the health information service in the main concourse to ensure that stroke information is current

Job responsibilities

  • To work as part of a cohesive team liaising closely with medical and nursing staff across the Trust
  • Setting professional standards for the junior team and maintaining them
  • To work alongside senior Stroke nurses and Medical team in assessing and caring for acute stroke pts
  • Work closely with ward nursing staff to ensure best evidence based practice is adopted through the development of care pathways, protocols and procedures related to the care of Stroke patients.
  • Assisting developing clear processes for staff regarding Stroke patient management in terms of timeframe for test, investigations, results, LOS discharge etc.
  • Be aware of any potential stroke patients for review in ED, on the MAU, or outlied on other wards
  • Identify new admissions and ensure that a plan is in place to bring outlying patients onto the stroke unit as soon as possible
  • Assess patient risk factors and advise on modifiable risk factors
  • Maintain and promotes privacy and dignity of all patients at all times.
  • Proactively seek to understand how improvements can be made to the service, and work with other members of the team to develop and implement these improvements
  • To work in accordance with the Trust policies and procedures.
  • To act appropriately as a senior nurse in emergency situations, and to be familiar with emergency procedures including fire and resuscitation.
  • To maintain and update registration with the NMC, including maintenance of a personal portfolio and revalidation, and to work within the scope of a professional practice as laid out by the NMC code of professional conduct.
