We are seeking a Senior Pharmacist to work clinically within designated teams in inpatient and training lead, acting as an expert source of pharmaceutical advice promoting safe, cost effective and evidence based prescribing and medicines use. In addition to the focus above, as part of the SHSC pharmacy team you will also work to provide specialist mental health pharmaceutical support to across the trust to help achieve best outcomes with medicines There will be a requirement to undertake on call support out of hours.

Main duties of the job

Advising on the pharmaceutical aspects of services provided to Single Point of Access (SPA) and in-patient mental health teams and other areas of SHSC business as required.

Taking a lead role in medicines training for newly qualified pharmacist and undergraduate pharmacists within the trust.

To lead on pharmacy practice to inpatient mental health teams within the Trust.

SHSC has EPMA (electronic prescribing and medicines administrations) throughout the trust In-patient sites.

Job description

Job responsibilities

To provide, develop and promote a responsive, patient focused, highly specialised clinical pharmacy service to patients and staff SHSC.

To deliver specialist information and expert advice to all members of the multidisciplinary team to maximise patient pharmaceutical care and ensure that medicines are prescribed, administered and supplied in a safe, effective and economic manner and to reduce harm caused by medication errors.

To provide direct supervision and training to clinical pharmacy staff, develop and provide training regarding the use of medicines and medicines governance principles to clinical staff, non-clinical staff, service users and carers.

To support the Deputy Chief Pharmacist in the delivery of all aspects of the Pharmacy Service.

To link closely with both inpatient teams and community teams ensuring a seamless care approach. Working in a multi-disciplinary team to ensure a holistic service is offered.

To undertake Non-Medical Prescribing according to currently held qualifications (if applicable) and Trust Policy. Reviewing and initiating pharmacological treatment within the scope of prescribing practice.

Participation in continuous professional development.

Participation in, and provision of, systems of clinical supervision and peer support within the pharmacy service.

Advising on the pharmaceutical aspects of services provided to SPA and in-patient mental health teams and other areas of SHSC business as required.
