An exciting new opportunity is available for a Registered Nurse to join our newly restructured Planned Integrated Care Service.

Planned/Unplanned service provide community Nursing services to Barnet CCG and is split into three locality teams. This post will be located in the West of the Barnet localities. The post holder will work closely with the team leads/ clinicians to develop the service and maintain close working relationships with both acute and community services.

You need to be compassionate person, able to show empathy for patients and their carers. Candidates must hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

Main duties of the job

The post holder will undertake community nursing duties involving delivery of nursing services and holistic assessments of patient's health needs using a person-centred approach.

This includes providing quality and seamless nursing care to housebound patients and being proactive in co-ordinating their care needs asappropriate, avoiding unnecessary admission to hospital or long-term care institutions.

In addition, the post holder will be involved infacilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery.

Person Specification



  • Registered Nurse with current UK NMC registration


  • Evidence of other post-registration education and training



  • Recent clinical nursing experience negotiating and liaising within a multi-disciplinary team
  • Working with people with complex health needs
  • Supporting students and other learners e.g. HCAs teaching others in the practice environment
  • Experience of undertaking general assessment by utilising evidence-based care
  • Previous experience in the Nursing process, care planning and fluent in record keeping
  • Recent clinical nursing
  • Experience of providing holistic care that involves the multidisciplinary team, negotiating and liaising within a multi-disciplinary team


  • Experience of training other students.
  • Participation in audit research projects service development
  • Post registration experience in a NHS community environment
