We are looking for a self-motivated registered nurse, learning disability nurse or professionally registered AHP to join our hospital-based learning disability and autism liaison team.

We are looking for someone who is an excellent communicator and enjoys problem solving, to promote and support a person-centred, seamless experience of healthcare services for our patients who have a learning disability and/or autism across the acute hospitals in the Trust.

The Learning Disability and Autism Acute Liaison Team are committed to reducing health inequalities through facilitating and promoting high quality, holistic healthcare for people who have a learning disability and/or autism.

You will have opportunities to support service development initiatives, deliver education and training for the workforce and provide direct support for our patients, their carers and families when they come into hospital.

The role involves working in partnership with a range of clinicians in a variety of teams both within and outside of the Trust.

You will play a key part in identifying change that is needed and there will be opportunities to support the team to facilitate these changes

Main duties of the job

To work within the acute Trust to provide expert guidance on the needs of people who have a learning disability and/or autism when they are accessing acute settings.

To support development of pathways and policies to facilitate equitable access to healthcare for this population group.

To ensure timely direct support for patients with a learning disability and/or autism, their family, carers and/or advocates according to clinical need and to liaise with all parties efficiently and effectively.

To promote the need for reasonable adjustments for this patient group across the acute trust, to assess individuals needs for person-centred reasonable adjustments and provide guidance and education to facilitate these.

To be an expert practitioner who can support colleagues and acute hospital staff to meet the additional needs of our patients in innovative ways.

To plan bespoke reasonably adjusted out-patient appointments for patients with complex needs across a range of specialities and provide clear communication for all involved.

The post holder will work within the hospital based learning disability and autism liaison team to develop the knowledge of the workforce within the Trust and provide supervision to students/learners and junior colleagues as required.

The post holder will act in accordance with the Nursing and Midwifery Code or the Allied Healthcare Professional Council Code of Conduct.

The post holder will be able to fulfil the NMC/HCPC requirements to maintain and revalidate registration.

The post holder is responsible for working with all professional groups throughout the acute trust services and partner agencies as appropriate.

They will support the management of care for people with a learning disability, autism or both.

The post holder carries continuing responsibility for the holistic assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care.

The post holder will provide specialist clinical care and advice to patients with a learning disability, autism or both, and to the professionals who support them.

They will contribute to the development of the Learning Disability and Autism Service.

The post holder will work with the Learning Disabilities and Autism Team to develop an increased specialist provision for patients and the people who support them.

The post holder will be responsible for direct and indirect patient care, in line with Trust policies.

To facilitate person-centred care across STSFT acute settings.

To provide Learning disability/Autism healthcare specific knowledge as part of the MDT

To contribute to the facilitation of the planning, delivery and evaluation of carepathways for people with learning disabilities and /or Autism

To contribute and facilitate in training and education within the specialist area.

• Work with adults with a diagnosis of a learning disabilityand/or Autism when they are accessing STSFT Acute services.
• To work closely with the existing Learning disability & Autism acute liaisonteam to increase the Learning disability & Autism liaison capacity across the acute trust centres.
• To engage, enable and empower others across the acute trust services to establish high-quality, compassionate, person-centred healthcare for our patients who have a learning disability or are Autistic.

Experience

Essential

• Demonstrable post registration experience at Band 5 or above (or equivalent), with experience of applying knowledge to a broad range of clinically and professionally challenging situations
• Relevant and recent experience of providing clinical support to people with a Learning Disability and Autism as part of a multidisciplinary team (MDT)
• Experience of clinical audit and standard setting
• Experience delivering training
• Experience of service user involvement
• Robust knowledge of the MCA/ MHA frameworks and experience supporting decision making in a patients best interests
• To understand the need for confidentiality in accordance with both the Data Protection Act and national / local guidelines
• Experience of working in a team, the ability to work independently with self-motivation and without direct supervision
