An exciting opportunity has arisen within the Bereavement Team at Walsall Healthcare NHS Trust. We have an opportunity for an experienced and motivated registered Midwife who is passionate about Bereavement care and Perinatal Mortality Review to join the team. The post holder will be responsible for co-ordinating and managing the PMRT (Perinatal Mortality Review Tool) Review process.
The post holder will be required to support the PMRT process through the inputting of data using the standardised review tool, and collate and present comprehensive timelines of events using a variety of sources. They will also support and co-ordinate the Perinatal Mortality Review Tool (PMRT) process by liaising with women / parents who have experienced pregnancy loss.

Main duties of the job
- Ensure all MBRRACE recommendations with regards to PMRT are implemented locally
- Work closely with the maternity governance team and the specialist bereavement midwife to support and facilitate the PMRT process
- Inputting of data into the standardized PMRT review tool
- Collate and present comprehensive timelines of events to be used to review individual cases
- To develop, disseminate and monitor action plans
- Manage your own time effectively and be able to prioritize to meet deadlines
- Lead the monthly PMRT Meetings, ensuring a quorate meeting of both internal and external reviewers.
- To support the PMRT process in line with the National recommendations and the Trust PMRT SOP
- Quarterly PMRT Reporting to the mortality surveillance group and the divisional director of midwifery

Patient Care Responsibilities
- The Lead PMRT Midwife will be an expert and have responsibility for ensuring that all eligible cases of late miscarriage, stillbirth and neonatal death are reviewed in line with national standards and local Maternity Risk Strategy using the MBRRACE Perinatal Mortality Review Tool, and be responsible for report writing to a high standard.
- Ensure that the PMRT review process is implemented to meet standards set by the NHS Resolution Clinical Negligence Scheme for Trusts.
- They will be responsible for working closely with the bereavement midwives to ensure all cases are identified for review and notified to MBRRACE.
- They will maintain responsibility for the organisation of PMRT multidisciplinary review meetings, and consideration of external agencies/trusts that may need to be invited to review meetings.
- Present cases at the multidisciplinary review meetings.
- Undertake critical assessment of cases through PMRT review process, whilst maintaining a safety culture.
- Build close liaisons with external agencies to ensure all information required for PMRT review is obtained.
- Work with the bereavement midwives and consultant body to ensure that families are informed of review findings. This might be by face-to-face meetings or in writing.
- Work closely with the Clinical Governance Midwives to ensure that learning identified from PMRT reviews is implemented into practice in a timely manner to support ongoing learning and improvement across the trust.
- Support data analysis and audit as required relating to bereavement and pregnancy loss.
- Engagement in review and dissemination of monthly and annual perinatal mortality reports.
- Work closely with the Womens Health Safety Team to ensure PMRT data is shared within the division and up to trust board, including development of the quarterly perinatal mortality report and the perinatal surveillance dashboard.
- Engage and lead on quality improvement projects to support embedding learning from PMRT.
- Work closely with the bereavement midwives to ensure that information required from PMRT is shared with the Child Death Overview Panel.
Professional Development and Education Responsibilities
- Complies with the regulations governing midwifery, particularly the Midwives Rules, Code of Practice, NMC Code of Practice and the Scope of Professional Conduct.

- Responsible for the accurate reporting of statistical information to MBRRACE and PMRT reports.
- Responsible for ensuring confidentiality and safekeeping of records for regards to women and staff in line with information governance and CQC requirements.

- To participate in implementing and reviewing policies and guidelines within the multidisciplinary clinical governance framework.
- Complies with and assists in implementing all existing Trust policies and guidelines. Guided by national pregnancy loss guidelines for Professionals issues by SANDS as well as by instructional documents produced by the Department of Health.
