As an Advance Choice Document facilitator, you will provide highly specialist interventions to work with service users who have previously been detained under the Mental Health Act to co-produce Advance Choice Documents (ACDs) with all the services they use across the Trust. The post holder will lead the development of co-produced, good, evidence-based practice for production of ACDs in mental health services and assist in delivering implementation strategies for their completion, access and use, including awareness raising and staff training.

The post holder will work across inpatient, crisis and community mental health teams to lead directly on the development of advance choice documents and to facilitate this process through awareness raising and training for all stakeholder groups: service users, cares/informal supporters, team staff and other staff whom service users wish to involve in the process such as peer supporters, advocates or support workers.

Main duties of the job

To apply a comprehensive range of communication skills with service users, carers, key supporters, multidisciplinary and Occupational Therapy colleagues and external agencies.To work as a member of the multi-disciplinary team, including the implementation of core decisions with regard to service users' care and treatment.To actively support and educate service users, carers, key supporters and colleagues regarding aspects of Occupational Therapy provision.To represent the service area/pathway on Trust-wide and national networks as appropriate.

To travel (e.g. home visits, community placements, meetings) as appropriate and across the Trust when required. To be aware of risk relating to aggressive and challenging behaviour amongst the client group and follow trust policies relating to its management.To respond appropriately and professionally to emotionally distressing situations (such as challenging behaviour, abuse etc) and to support others involved in such situations through Trust-wide approaches including Critical Incident Staff Support and Reflect-Support-Resolve.To work flexibly to meet service user needs.
