We are currently looking to recruit a full time Band 6 Mental Health Practitioner (Mental Health Nurse, Occupational Therapist or Social Worker) to join our small and supportive Community Mental Health Team for older adults, embedded within an exciting Integrated Care model (the Neighbourhood Team). This is a great opportunity to join our friendly and dynamic multidisciplinary team, with lots of support and CPD opportunities. There is free parking onsite.
We provide patient focused, evidence-based mental health interventions and assessment for people aged 65 and above, with a range of Functional and Organic mental health illness. You will manage a caseload of service users, delivering a range of clinical skills and therapeutic and recovery-based interventions. The work is varied and rewarding.
You will mainly be covering Job covers East Villages NT, Cambridge, will also require flexibility to cover other areas of Cambridge when required, so regular travel is required. Therefore, successful applicants will need to have a drivers License and access to a vehicle for work.
The post is based at Sawston Medical Centre. CPFT supports agile working, with flexibility for work-life balance. Visits to the team are welcome.
Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.
As a practitioner in the team you will act as a Care Co-ordinator for people who use the service and promote carer involvement in the delivery of care with service user consent.
You will manage a caseload of service users, delivering a diverse range of clinical skills and building a therapeutic and recovery based relationship. We seek to empower service users to actively engage in their recovery journey, working toward an effective discharge and utilising third sector support where needed.
The ideal candidate will have a strong team work ethic, sense of accountability, accompanied by an ability to work under pressure at times, and with a desire for innovation.

- Mental Health practitioners in Neighbourhood Teams will provide assessment and interventions for people with significant mental health conditions such as dementias, depression, psychotic illness and personality disorders.
- Offer training, advice and guidance to colleagues in the neighbourhood team on common mental health conditions.
- Participate in Neighbourhood Team MDT meetings and ensure key information is recorded in shared care plans Work closely with the mental health integrated care team in order to provide an excellent mental health service.
- Clinical Assessment of mental health needs.
- Assessment and management of risk.
- Participating in a duty rota to offer on call support and advice to service users and their carers. Holistic care planning and co-producing crisis plans with service users, keeping service users and their families at the heart of the intervention.
